Lactated Ringer’s solution is an isotonic sterile solution containing Na chloride 6 g/L, K
chloride 0.40 g/L, Ca chloride dehydrate 0.27 g/L, and Na lactate 3.17 g/L intended for
restoring electrolyte imbalance and water for hydration. Additionally, its Na lactate content
acts as an alkalinizing agent which normalizes the pH of the acid-base balance of the body.
Dose, frequency and route of administration (doctor’s order): (1 point)
IV Metacarpal at 125 mL/hr STAT afterbirth
Indication/s (reason/s) for administering the medication to the patient: (1 point)
This is given to the patient to help increase the fluid volume in her body to avoid dehydration

- This medicine may cause fluid overload and overhydration, which can lead to
pulmonary congestion (fluid in the lungs) and swelling. Report immediately if you
have a decrease in the amount of urine output, noisy, rattling breathing, trouble with
breathing, swelling of the fingers, hands, feet, or lower legs, troubled breathing at rest
- This medicine may cause hyponatremia (low sodium level in the blood), which can
lead to acute hyponatremic encephalopathy. Report immediately if you have loss of
consciousness, confusion, seizures, decreased urine output, dizziness, fast or irregular
heartbeat, headache, increased thirst, muscle pain or cramps, trouble breathing,
swelling of the face, ankles, or hands, or unusual tiredness or weakness.
- Lactated Ringer's injection may increase your risk of having hyperkalemia (high
potassium level in the blood). Report immediately if you have stomach pain,
confusion, irregular heartbeat, numbness or tingling in the hands, feet, or lips,
difficulty with breathing, or weakness or heaviness of the legs.

Oxytocin plays a vital role in labour and delivery. The hormone is produced in the
hypothalamus and is secreted from the paraventricular nucleus to the posterior pituitary where
it is stored. It is then released in pulses during childbirth to induce uterine contractions. The
concentration of oxytocin receptors on the myometrium increases significantly during
pregnancy and reaches a peak in early labor.22 Activation of oxytocin receptors on the
myometrium triggers a downstream cascade that leads to increased intracellular calcium in
uterine myofibrils which strengthens and increases the frequency of uterine contractions.
Oxytocin is one of the few that is regulated by positive feedback. The head of the fetus
pushing on the cervix signals the release of oxytocin from the posterior pituitary of the
mother. Oxytocin then travels to the uterus where it stimulates uterine contractions. The
elicited uterine contractions will then stimulate the release of increasing amounts of oxytocin.
This positive feedback loop will continue until parturition. Since exogenously administered
and endogenously secreted oxytocin result in the same effects on the female reproductive
system, synthetic oxytocin may be used in specific instances during the antepartum and
postpartum period to induce or improve uterine contractions.

Administration of exogenous oxytocin is indicated in the antepartum period to initiate or
improve uterine contractions for vaginal delivery in situations where there is fetal or maternal
concern. In the postpartum period, oxytocin may be used to induced contractions in the 3rd
stage of labor and to control postpartum bleeding or hemorrhage..

- Monitor the uterine activity and the fetal heart rate throughout the infusion of Pitocin.
Attention should be given to tonus, amplitude and frequency of contractions, and to
the fetal heart rate in relation to uterine contractions
- Discontinue the infusion of Pitocin immediately in the event of uterine hyperactivity
and/or fetal distress

- Oxytocin is a hormone that is used to induce labor or strengthen uterine contractions,
or to control bleeding after childbirth.
- Strictly follow doctor’s instructions about any restrictions on food, beverages or
activity.
