CONTRACT OF SERVICE

 
 
KNOW ALL MEN BY THESE PRESENTS:
 
This Contract of Service is made and executed by and
between:
 
         The LAO INTEGRATED FARMS INCORPORATED
(LIFI), a registered corporation created and established by
virtue of laws of the Philippines, with principal office at
Barangay Eman, Bansalan, Davao del Sur, duly represented
by Belle Jeanine S. Lao and Donna Rosemalyn Lao-Padre,
hereinafter referred to as the “First Party”.
 
-AND-
 
MARK STEPHEN CASTILLO, of legal age, married/single,
and a resident of, Bansalan, Davao del Sur, hereinafter
referred to as the “Second Party”.
 
 
WITNESSETH THAT:
 
1. The First Party is need of a __________________ and
for__________.

2. That the Second Party has signified his intention to be a

___________________, to which the First Party has accepted;

3. That the Second Party hereby possesses the education,

experience and skills required to perform the job as
described herein;

4. That the Second Party is to perform the following duties

and functions:

5. That in view hereof, the Second Party is hereby contracted

starting _________________, unless sooner terminated by the
______________ on any ground;
6. The Second Party agrees to perform his duties and
responsibilities to the best of his abilities and skills with
utmost care at all times and the highest interest of the
Municipality in mind;
7.  The Second Party shall not disclose to any person, firm or
corporation any confidential information concerning the
affairs of the First Party, which he may have acquired in
the course of or as an incident to this contract.

8. The Second Party shall render a total of Twelve (12) hours

in a week, divided by Four (4) hours in a day, of
supervision evidence by his signature on the log book, thus
Four (4) weeks in a month, which totals to Forty Eight (48)
hours each month.

9. Supervision fee shall be FORTY-FIVE THOUSAND (PHP.

45,000.00) per month unless modified by the Parties.

10. That the release of the Supervision fee shall be divided

into Two (2), on the 15th and 30th day of each month.

11. Failure to render the supposed hours of service in a week

shall result to addition to the following week of the
untendered payments of service.

12. Failure to render the supposed hours of the month shall

result to deduction of payments by its proportion.

13. The contract is valid for a period of ______________ or from

________________ until July ______________.

14. Clearly, it is understood that there is no EMPLOYER AND

EMPLOYEE relationship between the parties and that there
is no other law that will govern between them except the
provisions of this contract.

15.  Republic Act 9184 does not apply to herein Contract of

Services since the contract involves the engagement of an
individual to perform services where the selection and
evaluation process considers the individual’s qualifications
for the purpose of performing personal services.
 16. This contract shall be subject to the provisions of the
pertinent rules and regulations on contract of services.
 
 
         IN WITNESS WHEREOF, the parties have hereunto set
their hands this ____________________, at Bansalan, Davao del
Sur.

page where this acknowledgement is written has been signed
