GINA B. BIÑAN, of legal age, Filipino, married, and a resident of Purok Placida, Barangay
Calumpang, General Santos City, as my true and lawful attorney and representative to claim
from the Cashier of General Santos Doctors Hospital, Inc., the refund due me after payment of
my total bill from said hospital, and as well as sign any and all documents for the fulfillment of
said undertaking.

HEREBY GIVING AND GRANTING unto my said attorney-in-fact full power and

authority to do or perform any and all other acts and things necessary and proper to be done in
and about the premises as fully to all intents and purposes as I could lawfully do if personally
present, and HEREBY RATIFYING AND CONFIRMING all that my said attorney-in-fact or her
duly appointed substitute, representative or staff shall lawfully do or cause to be done in my
behalf by virtue of these presents, with full power of substitution.
