CONSOLIDATED CONSENT GREEMENT

For International Military Students (IMS) and Adult Accompanying Family Members (AFM)
The U.S. conducts international military education and training activities to promote defense
partnerships and the military readiness of both the U.S. and its partners. This document informs you of
necessary conditions to enable your participation in a Department of Defense (DoD) or U.S. military
training or education activity. Your acknowledgement and consent are necessary to comply with DoD
security policies and procedures to facilitate access to DoD or U.S. military bases and installations and
to increase security for both U.S. personnel and foreign military personnel. Your voluntary completion
of this form provides a means to document your consent and agreement for the duration of your
Invitational Travel Order (ITO):
1. You, and your adult accompanying family members (AFM), will not Transport, Possess,
Store, or Use Privately-Owned Firearms and Non-Official Ammunition in the U.S.
2. You will comply with Expectations of Conduct established by the applicable U.S. military
base or installation and U.S. civilian authorities.
3. You, and your adult accompanying family members (AFM), agree to be Monitored
through Publicly Available Electronic Information.

1. International Military Student’s Acknowledgement and Agreement to Abide by the

Department of Defense’s Prohibition on the Transport, Possession, Storage, or Use of Privately-
Owned Firearms and Non-Official Ammunition in the United States by International Military
Students, and their Adult Accompanying Adult Family Members

For the duration of my DoD issued invitational travel order, ,

I, Dainer Jair Vidal Vergara , agree to not transport, possess, store, or use a privately-owned firearm
(POF) or non-official ammunition in the United States not provided by the Department of Defense
(DoD) or Military Service explicitly for use in official IMS training. This includes agreeing to not
transporting a POF or non- official ammunition into the United States, not purchasing or possessing a
POF or non-official ammunition while in the United States, and not using another individual’s POF
or non-official ammunition on or off of DoD and U.S. military bases, installations and property.
Further, I acknowledge and understand that the above prohibition regarding POFs and non-
official ammunition applies equally to all of my accompanying family members who travel to the
United States pursuant to my DoD issued invitational travel order authorizing my family members to
accompany me. I agree that I am responsible for ensuring that all of my accompanying family
members comply with the above prohibition, including all of my non-adult accompanying family
members who are under eighteen years of age.
I have explained the applicability and requirements of the DoD prohibition concerning
privately-owned firearms and non-official ammunition to all of my accompanying family members,
including all of my accompanying family members who are under eighteen years of age but older
than twelve years of age, and I have obtained their agreement to abide by this prohibition. I have also
witnessed each of my accompanying adult family member’s affirm their voluntary consent by signing
this written agreement.
I understand that my violation or my accompanying family member’s violation of this
prohibition constitutes grounds for my immediate disenrollment from DoD and U.S. Military training
and education.

2. Expectations of International Military Students’ Conduct in the United States

Congratulations on your selection to participate in a United States military-hosted international military

training and education program. The United States places great importance on its defense partnerships
and on the longstanding benefits of these programs for the U.S. and its partners. It is our pleasure to
host our foreign partners’ finest military personnel to train alongside U.S military and civilian personnel
and those of other partner nations. As an International Military Student (IMS) and our guest, we also want
to ensure you understand your responsibilities while in the U.S.

As an IMS, you represent your home country to all who meet you. Accordingly, during your time in
the U.S. you are expected to conduct yourself with the highest degree of ethics and professionalism,
consistent with the Standards of Conduct followed by your U.S. military counterparts. The U.S.
government expects you to safeguard the health and safety of all military and civilian personnel in your
training program. This applies while you are on your training installation as well as off-base or on leave.
If you become aware of any condition or person that poses a threat to the community, you must
promptly report the matter to a proper authority who can take action to address the identified concern.
In other words, if you see something, say something. Your International Military Student Officer
(IMSO) will be able to provide you with installation-specific points of contact once you arrive in the
U.S.

A failure to adhere to ethical and professional conduct may have adverse consequences. You and your
authorized accompanying family members (if applicable) may be subject to civil or criminal
jurisdiction by local, state, or federal authority if you violate U.S. law. This communication does not
modify any civil or criminal jurisdiction provisions under an applicable Status of Forces Agreement.

 If applicable, you may be held responsible for your accompanying family members’ actions or
behavior. The Department of Defense encourages you to educate yourself and your authorized
accompanying family members of relevant federal, state, and local laws in the vicinity of the
installation where you will be attending training.

 You and your adult accompanying family members (if applicable) must comply with the standards
of conduct established by the U.S. military base or installation at which you are present. Failure to
adhere to the Department of Defense and the Military Service’s standards of conduct may result in
your disenrollment from training or other administrative actions.

 The Department of Defense and U.S. Military Services’ standards of conduct for IMS may
impose and expect adherence to academic standards, travel reporting requirements, and health
reporting requirements, and may also prohibit conduct that would reflect adversely on the U.S.
government, the Department of Defense, or the applicable Military Service.

 You should seek guidance from your assigned IMSO if you have questions regarding specific
standards of conduct at a DoD or U.S. military base or installation.

3. Consent to Monitoring of Publicly Available Electronic Information

For the duration of my DoD issued invitational travel order, ,

I, Dainer Jair Vidal Vergara , voluntarily consent to DoD and the applicable Military Services’ use
of publicly available electronic information (PAEI) pertaining to me to support initial and continuous
review of my fitness to access DoD and U.S. military bases and installations. I understand that PAEI
may include information from social media postings and any other electronic information published
or broadcast for public consumption, available by request to the public, accessible online to the
public, or accessible to the public by purchase or subscription. I understand that my refusal to
provide this consent constitutes grounds for my denial of access to DoD and U.S. military bases and
installations and may result in my disenrollment from DoD and U.S. military education and training.
I may record my decision not to provide consent by not signing and writing
“DECLINE” in the below signature block. I can provide my voluntary consent to the
described monitoring by signing this section.
