INSTRUCTIONS TO STUDENTS

(1) STUDENTS ARE ADVISED TO READ AND UNDERSTAND FULLY THE CONTENTS OF THIS DOCUMENT
BEFORE SIGNING.
(2) SHOULD YOU CONFORM TO THE CONTENTS OF THIS CONTRACT , PLEASE ATTACH A 1 ½ x 1 ½
COLORED ID PICTURE WITH PLAIN WHITE BACKGROUND, ASK YOUR PARENT/GUARDIAN TO SIGN, AND
SUBMIT TO THE OFFICE OF THE REGISTRAR.
(3) TAKE NOTE THAT THIS IS A BINDING DOCUMENT AND MAY BE ENFORCED AT WILL BY MALAYAN
COLLEGES MINDANAO TO ANY STUDENT WHO HAS BEEN FOUND IN VIOLATION OF ANY PROVISION OF
THIS CONTRACT.

MINDANAO as student, currently residing at
certify that:

I am fully aware that my admission to MALAYAN COLLEGES MINDANAO (hereafter referred to as the College), is
discretionary on the part of the college.

I attest that I am not involved in, nor a member of, any fraternity, sorority or organization of similar nature and purpose not
recognized by the college.

I commit myself not to join any fraternity, sorority or any other organization of similar nature and purpose not recognized
by the college during the entire duration of my study.

I acknowledge and understand that the following are considered as major offenses in the college :
a. Forming and joining any illegal organization such as fraternities and sororities not officially recognized by the
college, which includes its member students of the college;
b. Hazing, or infliction of any physical, emotional or mental harm or ordeal on any person, which act injures,
degrades, or disgraces, or that tends to injure, degrade, or disgrace any fellow student or any person in the
college. Harmful acts include, but not limited to, unwarranted initiations for admissions to any and similar
organization; and/or,
c. Physical assault upon or any threat to any member of the teaching and non-teaching employees,
administration, or any student, personnel or visitor of the college.

I fully recognize the following preventive measures and sanctions that may be imposed by the college on fraternities and
other student organizations of similar nature and purpose in consonance with CHED Memorandum Order (CMO) No. 4 series
of 1995 in order to deter violence:
a. Exclusion or expulsion of any fraternity member or student found guilty of:
i. Starting a fight;
ii. Being involved in a melee or other form of violence, except in self-defense;
iii. Carrying of knives, lead pipes, guns, and other weapons considered deadly and illegal to carry by
existing laws and statutes of the Republic of the Philippines in colleges and universities; and/or,
iv. Extortion;
b. Sixty (60) days suspension of all officers of any organization whether recognized or not whose members
have been found guilty of engaging in a fight or other violent acts with the knowledge of the said officers;
and/or,
c. One (1) year suspension of a recognized organization whose members have been found guilty of engaging in
a fight or other violent acts with the knowledge of their officers. For the second offense, the organization
will permanently lose their accreditation.

I commit to read and familiarize myself with the Student Rules and Regulations as contained in the Student Catalogue of
Information issued by the college to all its students, especially the Code of Conduct for Students while inside the campus .

I understand that the college has the right to dismiss any student who was found to have violated any provision of this
contract.

Finally, I declare that I understand all the provisions of this document and agree to its terms and conditions on free will with
out mental reservation or hesitation.
