UNDERTAKING

I, _____________________________________, a student to the Lyceum of the Philippines University hereby state

and acknowledge:

Please put an X opposite the appropriate statement (one statement only)

1. ______ I am not a member of any FRATERNITY / SORORITY / ORGANIZATION not recognized by the Lyceum of
the Philippines University;

______ I am a member of _______________________ FRATERNITY/SORORITY not recognized by LPU, but

hereunder is a written undertaking that I have accomplished and I am submitting to LPU.

2. For the duration of my stay in Lyceum of the Philippines University (LPU), I shall not join any fraternity and sorority not recognized
by LPU.
3. I acknowledge and understand that LPU has the right and the authority to choose the persons or individuals that may be admitted
as students; and,
4. I acknowledge and understand that LPU likewise has the authority to prescribe rules and regulations governing non-membership of
LPU students in any fraternity, sorority, or organization in LPU. The LPU Student Handbook states:

“Recruitment/membership in a fraternity/sorority or any student organization not recognized by the

Lyceum of the Philippines University (Very Serious Offense No.9, p. 56 of the Student Handbook)”

Sanction: Exclusion

“Hazing or subjecting a person to physical or mental injury for the purpose of admission and/or
maintenance of membership in any organization, whether recognized or unrecognized (Very Serious
Offense No.17, p. 57 of the Student Handbook.)”

Sanction: Exclusion

These rules are valid because they are based on the constitutional right of a school of higher learning to prescribe the conditions
they may require of any person or individual aspiring to become its student as well as public policy, as follows:

“The recognition by the Government of the right of schools of higher education such as Lyceum of the Philippines University to
withhold recognition to fraternities, sororities, or organizations whose existence is considered inimical to peace and order in
campuses.”

5. I recognize and accept that as stated in the Student Handbook, it is one of my responsibilities to abide by the school rules and
regulations. Failure to comply will warrant a disciplinary case against me.
6. I am executing and submitting this undertaking as a requirement for my admission/retention as a student of LPU.

By affixing my signature on this form, I hereby acknowledge and certify that I have carefully read and understood the terms and conditions of the Data Privacy Policy
of the Lyceum of the Philippines University (LPU). By providing personal information to LPU, I am confirming that the data is true and correct. I understand that LPU
reserves the right to revise any decision made on the basis of information I provided should the information be found to be untrue or incorrect. I likewise agree that
any issue that may arise I connection with the processing of my personal information will be settled amicably with LPU before resorting to appropriate arbitration or
court proceedings within the Philippine jurisdiction. Finally, I am providing my voluntary consent and authorization to LPU and its duly authorized representatives to
lawfully process my / my child’s data information.
