OATH OF UNDERTAKING
I, _____________________________________, of legal age, Filipino, Single/Married and
presently residing at _____________________________________________________________
___________________________________, after having been duly sworn to in accordance with
law, do hereby depose and state:

1. That I am presently enrolled in the Practicum (Off-Campus Learning) Course of

Bachelor of Science in Customs Administration (BSCA) at the Philippine Merchant
Marine School (PMMS), Las Piñas Campus for deployment to a reputable company as
my Host Training Establishment (HTE);

2. That Philippine Merchant Marine School shall implement the Minimum Public Health
Standards set by the government to Minimize the risk of the spread of Covid-19 in
conducting face-to-face classes;

3. That Philippine Merchant Marine School will ensure that the students that will be sent
to any Host Training Establishment (HTE) are cognizant and compliant with the
relevant Inter Agency Task Force (IATF) guidelines and have observed the proper
quarantine protocols so as not to disrupt the operations of the host company;

4. That under CHED Memorandum Order 30 s. 2021, as part of the curriculum of BSCA,
a minimum of at least 300 hours for Internship/Practicum is required, and divided into
two parts:

Part 1. Customs Brokerage Firm (at least 150 hours) and;

Part 2. Other Related Field such as but not limited to forwarding companies,
logistics companies, shipping/airline companies, and government offices (at least
150 hours).

5. That in the conduct of the limited face-to-face classes by the PMMS and on-site
internship/practicum by the HTE, I understand that my attendance in PMMS or at the
HTE is required and it will involve association with instructors, fellow students,
company employees, and school personnel inside and outside of the PMMS or HTE
that may put me at risk of Covid- 19 transmission, notwithstanding the precautions,
guidelines and procedures undertaken by the school and HTE;

6. That I acknowledge the fact that, Internship/Practicum activities that will be conducted
by the school is voluntary, and while there is a risk of possible Covid-19 transmission
to members of my family, I still take such risk and proceeded to attend activities set
forth by the school;

7. That my parent/guardian is allowing me to go through my internship/practicum at the

company that is recognized by PMMS as HTE;

8. That before I start my internship/practicum, I will attend the practicum orientation and
pre-deployment briefing conducted by PMMS;

9. That I will abide by the rules and regulation of PMMS and HTE during the duration of
the internship/practicum, any violation will have a corresponding penalty as stated in
the PMMS Internship/Practicum Manual;
 10. That I am fully aware of symptoms of Covid-19 such as fever, chill, cough, shortness
of breath or difficulty of breathing, fatigue, body or muscle ache, headache, loss of taste
and smell, sore throat, congested or runny nose, nausea, vomiting and diarrhea;

11. That at present, I am in good health. In the event that I feel any of the above- mentioned
symptoms, I will inform immediately PMMS, HTE and the person-in-charge that will
implement the protocols guidelines set forth by the CHED/LGU/IATF in dealing with
such dilemma;

12. That I hereby release and agree to hold Philippine Merchant Marine School (PMMS),
Las Pinas and other government agencies concern free from any causes of action,
claims, demands and compensation and understand further that this release discharges
the aforementioned from any liability both criminal / civil liabilities and whatever
causes of bodily injury, death and medical treatment.
