1. That I have completed the Classroom Instruction or academic requirements for

Onboard Training for the BSMT/BSMarE program Batch __________________
of Philippine Merchant Marine School (PMMS) – Las Piñas City.

2. That I am aware that I am required to undergo the Onboard Training as prescribed

by CHED-MARINA for the completion of my baccalaureate degree (BS) as per
compliance with the current CMO 67/70 guidelines and Memorandum Circular No.
1 s2019 and other similar circulars that will be issued by the Joint CHED-
MARINA.

3. That within 1 month of deployment by PMMS and partnered Shipping Companies

or by others, I or my family will submit COMPLETE and clear pictures of
documents enumerated below as part of student monitoring.
Furthermore, I UNDERSTAND THAT PMMS WILL REJECT UNMONITORED
SEAGOING SERVICE.

On Deferment of Deployment

4. That my refusal to communicate a status update or report to PMMS for

deployment within 5 months after Completed Classroom Instruction is
tantamount to forfeiture of my CAP funds, blacklisting and grounding of
any and all future BS application.

5. That continued deferment of my deployment beyond 18 months after

Completed Classroom Instruction up to a maximum of 6 years will likewise
subject me to a Refresher Course or Updating Course for new competencies,
knowledge understanding and proficiencies approved by CHED-MARINA.
 6. That if I defer my onboard training (for whatever reason), I will execute below
the detailed of circumstances/reason of why I am not onboard. This notarized
Oath of Undertaking will be part of my files with Onboard Training Office and
will be presented to CHED-MARINA monitoring group.
