MEMORANDUM OF AGREEMENT

Know All Men by These Presents:

This Memorandum of Agreement made and executed by and between:

FATHER SATURNINO URIOS UNIVERSITY, a non-stock, non-profit diocesan educational

institution established and existing under the Philippine Laws, with business address at San Francisco
Street, Butuan City represented by the Dean of the Accountancy Program, Dr. Evangeline P. Jamili,
CPA, hereinafter referred to as the FSUU.

The represented by its Branch Manager ________________,with office address at

________ herein after referred to as the TRAINER COMPANY;

WITNESSETH: That

WHEREAS, FSUU is a private institution offering (Your Course) ;

WHEREAS, The FSUU Bachelor of Science in Accountancy has 400 hours of Supervised
Industry Training (SIT) every First Semester 2021-2022 for students who are to be trained by experts
in different areas of Accounting and/or Audit;

WHEREAS, FSUU is presently in need of partner establishments that could further enhance the
necessary knowledge, skills and experience of its students taking up BS Accountancy.

WHEREAS, THE TRAINER COMPANY allows/agrees that the BSA students of FSUU
conduct/perform the training in their office/establishment;

WHEREFOR, in consideration of the foregoing premises and stipulations, the parties referred
herein affirm to strengthen the partnership in enhancing the necessary knowledge, skills, values and
attitude of students taking BS Accountancy course, specially, in the conduct of Supervised Industry
Training (SIT) every First Semester 2021-2022;

TERMS AND CONDITIONS

FSUU shall:

1. Assign (name of student), a BSA student-trainee (ST) to:

a. have the opportunity to work online, learn, and do Accounting and/or Audit-related
tasks in a real work setting;
 b. produce and present a paper containing their online experience and recommendations
to the management of the Trainer Company and FSUU Accountancy Program;
c. work part-time in establishments during off-class sessions as an apprentice on an
online arrangement
d. report for weekly conference in via Google Meet or Zoom at scheduled time, not
jeopardizing the other synchronous class schedules of the student;

2. Assign a Training Coordinator who will be responsible to monitor the student-trainee;

3. Monitor the performance of the trainee during the entire period of training;

4. Provide necessary technical assistance to the student-trainees and coordinate with the
manager of the Trainer Company while they are on training;

5. Ensure that student-trainees comply the necessary requirements set by the FSUU
Accountancy Program and the Trainer Company; and,

6. Ensure confidentiality of trainer company information gathered by the student-trainees.

Trainer Company _______(name of company)_________ shall:

1. Accommodate (name of student) a BSA student-trainee of FSUU and provide them with
relevant online work experience aligned with ACCOUNTING and/or AUDIT;

2. Provide the necessary guidance and technical assistance in all activities to be undertaken by
the student-trainee relative to the implementation of specified functions;

3. Confer regularly with the student-trainee, regarding the problems encountered and the
progress made during the training period;

4. Provide periodic feedback directly to FSUU through the assigned Training Coordinator in
order to create a healthy relationship;

5. Notify the Training Coordinator to pull out Student-Trainee for committing serious
infraction of the company's/industry's policies, rules and regulations. FSUU shall ensure
that the STs are aware of and follow all company rules and regulations, policies and
procedures, particularly those affecting the training activities, duly promulgated or to be
promulgated by the institution;
6. Not be held liable of any unavoidable circumstances or untoward incident during the
duration of the training beyond the control of the FSUU or TRAINER COMPANY after
having exercised proper precaution and guidance;
 7. Not to give tasks or workloads that are not related to Accounting and/or Audit;

8. Not to give tasks that would compromise health and safety;

9. The trainer company is welcome to attend the presentation of written output and participate
in the panel discussion or in the validation process.

Date of Effectivity, Renewal, Amendments, and/or Termination

This agreement shall take effect immediately upon signing of this document by the parties concerned
and shall remain in force until, unless amended or terminated by either of the parties with a 30-day
prior written notice to the other party.
