1. That I am the deponent of this Affidavit/Undertaking and I am well-conversant with the

2. That I am farmer and my annual income from this profession is approximately Rs. 230,000/-
(Pak Rupees Two Hundred and Thirty Thousand Only) and I have no other source of income.
3. That my son is unable to complete his education without Financial Support through National
Outreach Program at Lahore University of Management Sciences and I am also unable to
bear the hardship and cost of his education due to low income and financial constraints.
