1. TSH Levels are subject to circadian variation, reaching peak levels between 2-4 AM and the minimum between 6-10 PM. The variation is of the order of 50-206% Hence time of the day
has influence on the measured serum TSH concentrations (Reference:Tietz Textbook of Clinical Chemistry and Molecular Diagnostics - 5th Edition Page 123). Fluctuating TSH value must
be Clinically correlated.
2. Circulating TSH levels are known to show a circadian rhythm & diurnal variation. The diagnosis based on one TSH value which fluctuates is not reliable. Clinical correlation is
mandatory.
3. Values <0.03 ulU/mL need to be clinically correlated due to presence of a rare TSH variant in some individuals.

MRI BRAIN

STUDY PROTOCOLS:
FLAIR T1W AND FAST SPIN ECHO T2W HIGH RESOLUTION AXIAL IMAGES OF BRAIN WERE OBTAINED ON A HIGH
RESOLUTION DEDICATED PHASED ARRAY SURFACE COIL USING TWIN GRADIENT 16 CHANNEL HIGH DENSITY 3.0
TESLA SYSTEM WITH ZOOM GRADIENT COIL AND CORRELATED WITH T2W SAGITTAL,CORONAL, FLAIR AND
DIFFUSION AXIAL IMAGES. NO CONTRAST WAS GIVEN.

Clinical History: - Left upper and lower limb numbness

FINDINGS :

Cerebral parenchyma is normal in signal intensity with maintained grey and white matter differentiation. No diffusion restriction
is seen on DWI. No evidence of blooming is seen on GRE.

No acute infarct is seen.

Bilateral basal ganglia and thalami are normal in volume and signal intensity.

Ventricles are normal in shape, size outline and volume. Septum is in midline.

Basal cisterns and sylvian fissures are normal.

Sella and parasellar region are normal.

Brainstem is central and normal in signal intensity. Few small VR space are seen in the brainstem.

Fourth ventricle is central and normal.

Cerebellum is normal in signal intensity.

Major intracranial flow voids preserved.

Note is made of T2W hyperintense signal in the few bilateral mastoid air cells ? oto-mastoiditis.
Note is also made of mild pansinusitis with polypoidal thickening in left maxillary sinus.

IMPRESSION:- MR imaging reveals no significant intracranial abnormality.

ADVISED: CLINICAL CORRELATION.

EEG [ELECTROENCEPHALOGRAPH] REPORT

STUDY PROTOCOLS:
EEG has been done using internationally standard system for electrode placement.

FINDINGS:
It was an awake EEG record.

Background activity consists of 9 to 10 Hz sinusoidal alpha with occipital dominance bilaterally symmetrical and synchronous,
responsive to eye opening.

Occasional (5 to 6 Hz) slow wave activity seen for a brief period of time.

No focal/generalized epileptiform discharges seen.

Photic stimulation and hyperventilation are non contributory

IMPRESSION: This awake EEG record is borderline abnormal, showing occasional background slowing for a brief
period of time.
