RELEASE OF LIABILITY AND DATA PRIVACY CONSENT WAIVER

By agreeing to this document, I hereby release DCI/I, its subsidiaries and management representatives, stockholders, directors,
officers and employees from any liability arising from or relating to any loss, damage to personal property and/or injury sustained
by myself during the DCI/I 2022 GSA 2 Company Outing on May 14-15, 2022 at Bataan White Corals Beach Resort, Sitio
Panibatuhan, Brgy. Poblacion, Morong Bataan and Zoobic Safari, Group 1 Ilanin Forest Forest Adventure Park Subic Bay Freeport
Zone, 2222 Olongapo, Philippines. I will assume full responsibility for the consequences of any actions that may lead to the loss
or damage of personal property or inflicted injury to myself during the Event.

I hold DCI/I free of any liability in the event of damage to my personal property and/or injury sustained by myself during the event
as a result of any alcohol-related or induced actions. I waive the right to pursue any legal claim against DCI/I and I will not deem
DCI/I, its executives and management representatives, responsible for any damage or harm that may arise from my participation
in this activity. I will assume full responsibility for the consequences of any action that may lead to the loss or damage of personal
property or inflicted injury to myself.

Further, I accept and understand that the DCI/I Code of Discipline and Ethics remains in full force and effect all throughout this
Event and wherever applicable, in schedules or locations where the Code of Discipline and Ethics remains applicable.

CONSENT AND RELEASE – Upon agreeing to this electronic document, I hereby consent to the recording, reproduction, and
unrestricted use of my image and/or likeness in whole or in part, in any form whatsoever, by DCI/I or any of its subsidiaries for any
and all publication necessary. I also hereby release DCI/I and its subsidiaries, shareholders, officers, directors and employees
from any and all claims, demands or liability of any and every nature arising out of such use and by this instrument bind my heirs,
executors, administrators and assigns.
