I HEREBY acknowledge, agree, and represent that:

Non-Political Affiliation Declaration

“I understand that Ventureslink Management Solutions, Inc. is contracted to provide services in relation to the automated National
and Local Election on May 9, 2022. That I fully understand that the work engagement offered by Ventureslink Management
Solutions, Inc. requires non-affiliation to any political candidate or parties, national or local.

I HEREBY declare that I am not affiliated with any political candidate of parties, national or local, starting from myself to the 4th
degree of consanguinity and affinity of relations.

THAT I am executing this statements to attest and declare that the foregoing facts as true and correct.”

Non-Disclosure Agreement

“I understand and agree to receive from Ventureslink Management Solutions, Inc. information in confidence and to treat it as
confidential for all purposes. That I will not divulge or use in any manner any of the said confidential information unless by written
consent from Ventureslink Management Solutions, Inc., and I will use the efforts it regularly employs for its own confidential
information to avoid disclosure to others.

That I will only use confidential information for the purpose of the implementation of the project assigned to me that is necessary to
fulfil my contractual obligations or to complete or fulfil the specific task, assignment or purpose for which the Company discloses the
information in furthering the Company's business operation. That I will not duplicate or reproduce copies unless authorized to do so.
I will exercise my best efforts to conduct discretion in handling all entrusted to me and I will return all information, including all copies
thereof, to Ventureslink Management Solutions, Inc. upon request.

I also understand and agree that in any case of my violation of this Non-Disclosure Agreement, the company is entitled to all
remedies available at law, to seek and obtain from any court of competent jurisdiction temporary, preliminary and/or permanent
injunctive relief as well as any other relief permitted by applicable law.

This agreement shall remain in effect after the date of its execution, and it shall be construed, interpreted and governed by the laws
of the Philippines.”

Data Privacy Consent

In compliance with the Data Privacy Act (DPA) of 2012, and its Implementing Rules and Regulations (IRR) effective since
September 8, 2016, I allow Ventureslink Management Solution Inc. (“VMSI”) to provide my information to its clients in relation to
employment. As such,

I agree and authorize VMSI to:

1. Continue to use my information to process employment services.

2. Retain my information for a period of seven (7) years from the date of termination of my employment or at such time that I
submit to VMSI a written cancellation to this consent, whichever is earlier and I agree that my information will be
deleted/destroyed after this period.
3. Retain my information in the VMSI Database shared with other companies in accordance with the manpower services
agency.
4. Share my information to affiliates and necessary third parties for any legitimate business purpose. I am assured that
security systems are employed to protect my information
5. Acknowledge and warrant that I have acquired the consent from all parties relevant to this consent and hold free and
harmless and indemnify VMSI from any complaint, suit, or damages which any party may file or claim in relation to my
consent.
