Important note for successful booking of COVID-19 vaccine:


On the day of vaccination, you should
bring the following documents: (i) original identification documents ; (ii)
priority group document proof (if applicable); and (iii) proof of successful
booking of the booking system.
If you (or your carer(s)*) are between 12 - 17 years old and booking for receiving
BioNTech vaccine, please download and complete the consent for vaccination by
parent/ guardian and bring it along to vaccination.
If you (or your carer(s)*) are between 5-11 years old and booking for receiving
BioNTech vaccine, please download and complete the consent for 5-11 years old for
BioNTech vaccination by parent/ guardian. On the day of vaccination, please bring
the signed consent along and be accompanied by your parent / guardian to the
Community Vaccination Centre. If the identity document of the child (e.g. birth
certificate) does not bear a photo, please remember to bring the child’s latest
student’s handbook or school record bearing the child’s name, date of birth and
photograph, or other identity document with photo to facilitate the healthcare
staff to verify the identity of the child.
Wear clothes that the upper arm can be exposed easily for vaccination.
For children and adolescents receiving the BioNTech vaccine, the vaccine is
suggested to be administered in the muscle of the mid-anterolateral thigh. They
should wear loose-fitting clothing for ease of vaccination.
Please be aware of the following situations:
if you are feeling unwell on the vaccination day, such as having fever or cough,
please do not visit the community vaccination centre or clinic and seek medical
attention promptly.
If you are in doubt on the suitability for vaccination due to your background
medical illnesses, please discuss with your doctor.
If you want to know more about the suitability, efficacy, side effects and the
potential adverse reactions following vaccination, please visit the website at
www.covidvaccine.gov.hk.
If you have agreed to join eHealth when you made this appointment booking, please
express your will to the clinical staff when you receive vaccination.
Citizens going to the vaccination centres should also comply with the visitor
infection prevention measures of the individual venue.
The final arrangement of vaccination will be arranged by the staff of the community
vaccination centre or clinic based on the programme arrangement and the actual
situation.
