AFFIDAVIT OF UNDERTAKING

using my personal Facebook account between me and that of my interviewer on

3. That due to the privacy and confidentiality required by ethical standard in the
practice of profession by the interviewer, I prohibit myself to any video and audio
recording, picture taking or any way that the interview be heard by the person/s;

4. That any item taken from the above-cited circumstances be more aggravating if
these are reproduced, shared, talked-with or any possible means that any content
of the interview is leaked;

5. I fully understand that any violation of this undertaking would render me criminally,
civilly and administratively liable pursuant to Republic Act No. 4200 as amended or
otherwise known as the Anti-Wiretapping Law, and other pertinent laws; and
