JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

That we, ANALY G. SARONO, of legal age, Filipino, and a resident of Maningcao, Sibulan, Negros
Oriental, and JEFONIE A. SARONO, of legal age, Filipino and a resident of Maningcao, Sibulan, Negros Oriental,
under oath, do hereby depose and manifest:

That we personally know the person of “TALINO CADA OLPOS”, and that recently we came to know
that his name as indicated in his Certificate of Marriage was “TOLINO CADA OLPOS” and SSS Membership
Details (E-1) was “TALENO CADA OLPUS” and we know for a fact that the person of “TOLINO CADA
OLPOS” as appearing in his Certificate of Marriage and “TALENO CADA OLPUS” as appearing in his SSS
Membership Details (E-1) and the person of the name “TALINO CADA OLPOS” as appearing in his Death
Certificate, REFERS TO ONE AND THE SAME PERSON, the latter being real and correct name.

That we also personally know the person of “ROSALINDA GONZALES OLPOS”, and that recently we
came to know that her name as indicated in her SSS Membership Details (E-1) was “ERLINDA GONZALES
OLPUS” and we know for a fact that the person of “ERLINDA GONZALES OLPUS” appearing in her SSS
Membership Details (E-1) and the person of the name “ROSALINDA GONZALES OLPOS” as appearing in her
Certificate of Marriage, REFERS TO ONE AND THE SAME PERSON, the latter being real and correct name.

That we also personally know the person of “ENOBEJES E. CADA”, and that recently we came to know
that her name as indicated in her SSS Membership Details (E-1) was “GENOBEJES CALUMBA OLPUS” and
we know for a fact that the person of “GENOBEJES CALUMBA OLPUS” appearing in her SSS Membership
Details (E-1) and the person of the name “ENOBEJES E. CADA” as appearing on TALINO CADA OLPOS and
ROSALINDA GONZALES OLPOS Certificate of Marriage, REFERS TO ONE AND THE SAME PERSON,
the latter being real and correct name.

Perhaps, the incorrect entry was due to honest mistake and inadvertence. That we know of these facts
considering that we are acquiantances of his, and that neither of us is related with him either by affinity or
consanguinity and that we are executing this joint affidavit freely and voluntarily in order to attest to the veracity of
the facts above-stated and for whatever legal purpose this may serve best;
