FINAL REPORT
This is to report that the undersign Marine Surveyor of Boyd Marine Consultants did, at the
request of SPICA Services Indonesia on behalf of MITSUI SUMITOMO INSURANCE
COMPANY and their Members, ASIAN SHIPPING S.A, attend on board bulk carrier MV.
“FORTUNE BAY” (the “Member’s Vessel”) at jetty# 3 IMIP of Bahodopi Port, Morowali,
Central of Sulawesi – Indonesia on 29 th December 2020 for pre-loading and loading
supervision of steel billets and coils cargo as well as conducting tally survey in order to
protect Club/Member’s interests.
1.1 Upon receiving your email instruction on 26 th December 2020 at 2106 LT, we
subsequently liaised with Mr. Ervin as local agent at Bahodopi who informed
the Member’s Vessel will arrive at Bahodopi anchorage on 28 th December at
0100 LT.
1.2 Subsequently, on the following day we dispatched from base for proceed to
Bahodopi port by flight on 28th December at 1000 LT and safely arrived at
Kendari airport at 1130 LT then continue headed to Bahodopi by land
transportation and safely arrived at 2000 LT.
1.3 On 29th December at 1200 LT, we liaised with Mr. Purnomo as foreman that
the Member’s Vessel will alongside at the jetty No. 3 on the same day at
2000 LT.
1.4 On the same day, at 1800 LT we re-liaised with Mr. Purnomo as foreman
that the Member’s Vessel will alongside on the following day 30 th December
at 0500 LT.
1.7 The cargo loading steel coils and slabs operation had fully completed on 31st
December 2020 at 0415 LT.
1.8 On 1st January 2021 at 0908LT, we received Documents Complete from Mr.
Ervin as local agent.

4.0 PRE-STOWAGE PLAN
Based on gathered information and refer to the loading plan and pre-stowage plan
available at time of our attendance, we noted as follows: -

Total loading cargo steel coil into the holds on 31 st December 2020 at 0415 LT as
follows: -
The pre-shipment inspection at Shipper’s Warehouse had not been conducted since
not permitted by Shipper to conduct survey at their warehouse, instead of the pre-
shipment was carried out on the wharf due to the cargo only made available for

inspection during loading and the inspection could only be conducted prior to the
cargo being transferred onto the ship’s holds and we noted as follows: -
6.3 The cargo steel coils and steel slabs were transferred from warehouse by
means of truck transported to the ship side, and cargo of steel coils on the
truck was covered using tarpaulin during transferred while the cargo of steel
slabs was uncovered with tarpaulin.
6.4 The cargo of steel coils and steel slabs at ship side was lifted up from the
truck to ship holds using shore crane. And the cargo stacked in holds using
forklift and assist by labors.
6.5 The wooden boards in various size were placed between stacks of cargo and
tank top plate by dunnage, several wooden boards added as chocking which
placed vertically for protection between cargo and ship’s plate. And the
lashing / securing of cargo using steel band clamping which was carried out
by Stevedore.

The above shipments were carefully inspected by us prior to and during loading
operations and following damages were found during loading cargo operation of the
cargo steel coil with green packed found damage slightly scratched, crumpled and
torn at several place of the packed cargo before shipment, as follows: -

10.0 The cargo steel coil and steel slabs was transferred directly from shipper
warehouse using truck trailer. During transferring from warehouse to wharf
the cargo was covered with tarpaulin, while the cargo of steel slabs was
uncovered with tarpaulin.
10.1 The silver nitrate test randomly on the cargo presented onboard for loading
especially to the cargo which had found to have sign/traces of rusty

condition on the surface, however the result of test was found negative
reaction which indicated the cargo to be unexposed or uncontaminated with
chlorides.
10.2 The wooden boards in various size were placed between stacks of cargo and
tank plating as dunnage, several wooden boards added as chocking which
placed vertically for protection between cargo and ship’s plate.
10.3 Numerous of the cargo of steel coils with package has been found slightly
scratched, dirty, dusty, torn, crumpled on several places before shipment.
10.4 The cargo stacked in holds using forklift and assist by labors. And the
lashing / securing of cargo using steel band & clamping.
10.5 Throughout loading operation, the weather sometimes found in small rain
condition.
10.6 During small rain, the CH No.1 & 5 was in closed condition while CH 2, 3,
and 4 keep continued for loading progress.
10.7 Upon completion of cargo loading, we provided to Master and C/O with
written suggestion on clausing the Mate’s Receipt and or B/L. the suggestion
and cargo damage list was acknowledged and signed by Master.
