Instructions for Visitor’s Portal and Online Health Declaration Form

1. Log in to the Visitor’s Portal. (Recommended browser: Chrome)

Click Register.

2. Activate your account by clicking the link sent to email after registration.

3. Proceed to Account Registration.

Create an account by filling out the registration form.

4. Again, Go to Visitor’s Portal then click Sign In.

5. Click and access the Health Declaration Form and fill out needed information

6. After Health Declaration form, click and fill out Vaccination status.
