This is to inform you of our intent to implement Limited Face to Face Classes
particularly to Kindergarten to Grade III this coming January , 2022 for the following
reasons:

1. Provide better delivery of education among learners

2. Zero COVID case/s in the community
3. Stakeholders’ support in the implementation of Limited Face to Face Class
4. Number of learners per grade level is manageable

Further the school prepared the necessary requirements for the implementation
of the Limited Face to Face classes in the line with the IATF Protocols and Standards
set in DepEd-DOH JMC No.1 s. 2021 to ensure safety of learners and teachers.

MEMORANDUM OF AGREEMENT

KNOW ALL BY MEN BY THESE PRESENTS:

This Memorandum of Agreement entered into and executed this _____ day of
_____________ by and between:

ENRILE WEST CENTRAL SCHOOL with school address at Babaran Street,

Barangay San Jose, Enrile, Cagayan, represented by the School Principal
REYNOLD D. TUNGCUL,hereafter called as the “FIRST PARTY”;
- AND -
The LOCAL GOVERNMENT UNIT OF ENRILE, a local government unit
existing under the laws of the Philippines herein represented by Mayor
MIGUEL B. DECENA JR., with office address at Municipal Hall Roma Norte,
Enrile, Cagayan hereafter referred to as the “SECOND PARTY’;
- - WITNESSETH –

WHEREAS, the President has approved the pilot implementation of limited

face to face classes in various areas and regions in the country.

WHEREAS, the Department of Education and the Department of Health

issued a Joint Memorandum Circular No. 01, s. 2021 prescribing the
operational guidelines of limited face-to-face learning modality;

WHEREAS, based on the Joint Memorandum Circular, one of the conditions

to be satisfied by schools that will be included in the pilot phase is the written
concurrences or support of the LGU concerned.

WHEREAS, Enrile West Central School in the Municipality of Enrile is already

prepared based from the survey conducted using the School Safety
Assessment Tool;

WHEREAS, recognizing the necessity of face to face classes in providing

quality education, the Local Government Unit of Enrile is giving its full
support and assistance to Enrile West Central School to be included in the
pilot phase;

NOW THEREFORE for and in consideration of the foregoing premises, the

parties hereby agree to the following:

THE FIRST PARTY SHALL:

1. Ensure that it passed all the criteria provided under the School Safety
Assessment Tool upon validation of the Department of Education;
2. Ensure that its teachers and school personnel are fully vaccinated. All
personnel who are not vaccinated shall be coordinated to the Second
Party.
3. Exert efforts to ensure that parents of students participating in limited
face to face classes must be fully vaccinated. Those who are not
vaccinated shall likewise be coordinated to the Second Party.
4. Ensure the strict implementation of health and safety protocols during
the conduct of face-to-face classes and implement precautionary
measures to protect its teachers, personnel, and learners.

THE SECOND PARTY:

1. Provide full support and assistance to the First Party by mobilizing its
BHERTS, barangay officials and health personnel in ensuring the safety
of learners and school personnel.
2. Establish referral system for COVID 19 suspects, probable, and positive
patients.
3. Organize Contact Tracing Team per school or district to facilitate
prevention, detection, isolation, and treatment of infected patients.
4. Establish an information dissemination system that updates the
students, parents, and school personnel about the evolving situation
and measures taken in the event of COVID 19.

This agreement shall take effect on the date of its signing and shall remain to
be in full force and effect until further modified and revoked. Any amendment
or modification may be made upon mutual assent of both parties.
