MEMORANDUM OF AGREEMENT

This Memorandum of Agreement is entered into by and between the following:

The TESDA Samar Provincial Office with official address at Government Offices
Barangay 7, Catbalogan City, Samar herein represented by its Provincial Director, LORETA
D. BANARIO, hereinafter referred to as the FIRST PARTY;
-AND-
The City Mayor of the City of Calabayog, with official address at City of Catbalogan,
Samar, herein represented by its City Mayor, HONORABLE RONALDO P. AQUINO herein
referred to as the SECOND PARTY;
Witnesseth
WHEREAS, Republic Act 7796 mandates TESDA as the lead government agency to manage
and supervise quality technical vocational education and training in the Philippines;
WHEREAS, in the light of the COVID 19 pandemic, the expansion and increase of health
related qualification is critical to produce more equipped and competitive health practitioners
and effectively respond to such crises;
WHEREAS, the implementation of programs and development of Trainers and Assessors in
Barangay Health Services will augment the healthcare workforce and contribute to the health
security in the communities;
WHEREAS, in line with the agency’s efforts to serve the people and support government efforts
on preventing COVID19 and possibly augment healthcare workers needed, all the TESDA
Technology Institutions including Provincial Training Centers are hereby instructed to register at
least one healthcare related program based on the existing training regulations;
WHEREAS, TESDA Technology Institutions and Provincial Training Centers may tap health
institutions or barangay health centers for the facilities, tools and equipment needed for the
qualification;
WHEREAS, in view of the urgent need to respond to this requirement, TESDA Samar
Provincial Training Center will partner the initial implementation of the program with the City
of Calbayog through its City Health Office;
NOW, THEREFORE, for and in consideration of the foregoing premises, the parties hereby
mutually agree to undertake the collaborative partnership in accordance with the following terms
and conditions:
Obligations of the First Party:
1) Provide the qualified trainer that would handle the program in Barangay Health Services
NC2 in the City of Calbayog by qualifying the prospective trainers through certification
and Trainers Methodology Training;
2) Implement the program in the City of Calbayog with the beneficiaries coming from the
said municipality;
3) Submit to the Regional Office the enrolment and graduates reports through the T2MIS;
4) Ensure that the graduates of the program are subjected to mandatory competency
assessment;
5) Regular monitoring during the conduct of the training program;

Obligations of the Second Party

1) Provide the required facilities, pieces of equipment, tools and consumables during the
conduct of the training program on Barangay Health Services NC II;
2) Provide the training venue for the conduct of the training program;
3) Provide Samar Provincial Training Center with the list of beneficiaries/prospective
enrollees to the said training program;
4) Regular monitoring during the conduct of the training program in coordination with
TESDA
EFFECTIVITY AND AMENDMENTS:
This Memorandum of Agreement shall take effect upon signing by the representatives of
the parties and shall remain in force unless terminated through mutual consent of both
parties. Any amendments to the provision of this memorandum of agreement shall be made
in writing with the consent of both parties;
