1. Purpose and Scope


This JSA is specific for pre-commissioning and commissioning of 33KV GIS system &
CRP panel, 11KV, 400V and all other panels onshore section of RAS MARKAZ CRUDE OIL
PARK PROJECT PHASE 1.

2. Definition and Abbreviations

Project Name : RAS MARKAZ CRUDE OIL PARK PROJECT (PHASE 1)


EPC Contractor : China Petroleum Pipeline Engineering Co., Ltd. (CPP)
Sub-contractor : Means any person named in this EPC Contract as a
Subcontractor, or any person appointed as a Subcontractor (of
any tier), for a part of the Works, and the legal successors in title
to each of these persons.
JSA : Job Safety Analysis
PTW : Permit to Work
RAM : Risk Assessment Matrix
HSSE : Health, Safety, Security and Environment
OHS : Occupational Health and Safety
GIS : Gas Installation System

3. Instructions for Completing This Form


A blank JSA Form shall be used for each work activity. Appendix 1: Job Safety Analysis Form

3.1 Section Ⅰ. Enter general information about the site and job task.

1. Enter the location, date, JSA No. as well as the PTW No.

2. JSA members shall sign the JSA and approved by permit issuer.

3. The Site Manager/HSSE Manager/Supervisor of Contractor shall review the JSA prior to
starting the job and sign this form.

4. Identify the equipment and personnel competency details.

5. Details of Special Precautions to be taken prior to job commencing. The site


Manager/HSSE Manager/ Supervisor shall specify additional precautions in the form after
the review when needed.

3.2 Section Ⅱ. Job Safety Analysis

STEP 1: Plan out the TASK SEQUENCE - What are you going to do?

STEP 2: Identify the HAZARDS – What could cause harm or damage?

STEP 3: Identify the TOP EVENT – What is the most serious impact caused by the hazard
cause harm or damage?

STEP 4: Identify the on sequences– What is the HSE effects & impact?

STEP 5: Identify who or what may be harmed

STEP 6: Select your BARRIER / CONTROLS – What will be put in place to keep you safe?

STEP 7: Recovery

STEP 8: Allocate the RESPONSIBLE PERSON – Who will do this and how to recovery

STEP 9: Finalise the JSA

STEP 10: Site responsible persons, such as Foreman and HSSE officer, shall REVIEW AND
MONITOR the JSA during the course of the work.

Carry out task as per plan. If any conditions change ---STOP & RE-ASSESS.

Once JSA form (Appendix 1: Job Safety Analysis Form) is completed, it is attached to the
original copy of work permit available at site. On completion of the job, the JSA Form shall
be retained in file with Permit Issuer as a record.
