Game, hereby certify that I have examined Ms. JAENA KATE SICAT, 39
years of age, and a member of the RP Dragon boat team.

As per my diagnosis, she has no disease, injury, constitutional affliction, or

bodily infirmity as well as no personal history incapacitating her to undergo
the professional training. Also, on clinical examination, she is deemed
medically and physically fit to perform in the Dragon Boat Race Women’s
Division for the 31st Southeast Asian Games.

Certified further, that she has not shown any evidence of major defects of
posture, locomotion, vision, hearing, or any other systemic disorder.
