PROGRAM EXTENSION REQUEST FORM
This form is used to request an extension of time beyond the university’s established time limits for degree completion. The
student advisor, department graduate committee (DGC), college academic commission (CAC), and academic affairs vice
president (AAVP) or director of graduate programs (DGP) must approve extension request. students who incur an excessive
number of extensions and who don’t make progress toward their degrees and subject to dismissal or forced withdrawal.
