PIPE DISLODGED FROM PIPE CLAMP AND HIT A WORKER

BRIEF DESCRIPTION OF THE INCIDENT

On 14th June 2022 at approximately 1215 hrs, a fitter (IP) was conducting a fit-up activity
for a 36'' pipe. While performing the task, the clamped pipe dislodged from pipe clamp and
hit IP’s torso. The IP experienced pain on the shoulder and shortness of breath. IP was
immediately sent to Klinik Kesihatan Kemasek and subsequently brought to Hospital
Kemaman for further assessment.

The work was stopped immediately and safety stand down was conducted at site.

RECOMMENDATIONS

Pending completion of the investigation, we strongly recommend the following preventive


measures to avoid similar incidents:

1. Be vigilant on the presence of Line of Fire and avoid being in the Line of Fire e.g. under
suspended load, in a position where there is a possibility of being hit by object under
force (tension) or moving object.

2. Ensure supervision of work and always work as a team i.e. observe team mates position
and ensure they are not in the Line of Fire.
