1.0 INTRODUCTION

1.1 OBJECTIVE

A Lifting Operation Plan (LOP) for the purpose of establishing the minimum requirements to be
performed during the Critical Lifting of HWU / Equipment / Cargos.

1.2 SCOPE OF WORK

a. A detailed JSA and toolbox talk is to be conducted and documented prior to the operation starting.
b. Locate all of the equipment as per the SIT Layout drawing.
c. Connect the lifting sling assembly to the top of the HWU / Equipment / Cargos and connect to the
main crane.
d. Connect an additional, suitably rated sling to the bottom of the HWU / Equipment / Cargos and
attach to the second crane.
e. Connect the 4 x retaining wires to the bottom corners of the HWU / Equipment / Cargos. Monitor
for snagging during the lift.
f. Confirm that tag lines are fitted in suitable places on the equipment and that all personnel are
standing clear of the lift.
g. Commence taking the tension at the top of the HWU / Equipment / Cargos with the main crane.
h. The second crane will also take tension on the bottom of the HWU / Equipment / Cargos as the
main crane does, but will not commence lifting.
i. Continue to lift using the main crane, keeping tension on the second crane to maintain the lift off
the ground.
j. As the main crane lifts, the second crane will commence booming down and maintaining the lift at
a constant height from the deck.
k. Constant tension should be kept on the tag lines during the lift and excellent lines of
communications should be ongoing at all times.
l. Once the main crane has the HWU / Equipment / Cargos vertical, slowly release any tension
remaining on the second crane to allow removal of the rigging.

1.3 DEFINITIONS

RA Risk Assessment - a detailed assessment of the threats apparent in
a lifting task and the controls required to reduce or eliminate those
threats
SWL Safe Working Load - the maximum load which an item of lifting
equipment may raise lower or suspend under a particular service
condition
WLL Working Load Limit - the maximum load which an item of lifting
equipment may raise lower or suspend
RCI Rated Capacity Indicator – A safety device fitted to a crane hoist to
alert the operator when the crane is approaching the maximum
allowable load and when the crane has exceeded the allowable
maximum load
CoG Centre of Gravity _ a determination made during lifting to prevent
over moment or undue movement of the load

FoS Factor of Safety - a margin of safety applied to a particular item of


lifting equipment utilising a factor determined by the appropriate
manufacturing standards
MF Mode factor - a factor applied to single items of wire rope slings and
fibre slings to determine the minimum SWL of the item
AF Angle factor - a factor applied lifting appliances to determine the
minimum SWL of the item when used at a prescribed angle
RF Reduction Factor - a percentage factor applied to items of lifting
equipment to determine the minimum SWL of the item
SF Skew Factor - a factor applied lifting appliances to determine the
minimum SWL of the item when the possibility of skew (unequal
loading exists)
EF Enhancement factor - a factor applied to the lifting set of a CCU to
determine the minimum required SWL of the item
CCU Cargo Container Unit - an item of lifting equipment utilised to contain
and transport goods and cargo to and from a supply base and an
offshore installation
MBL Minimum Breaking Load - a breaking load applied to an item of
lifting equipment to determine strength, usually performed by the
equipment manufacturer as a type test.

Tare Weight The un-laden weight of a CCU
Payload The maximum permissible weight that a CCU can carry
Gross Load The maximum allowable weight of a CCU. Tare Weight plus
maximum Payload
Certification A certificate that applies to any particular item of lifting equipment.
The certificate can be awarded as an MBL test, a proof test or as a
certificate of conformity.
Thorough Examination An examination of lifting equipment by a competent and impartial
lifting equipment surveyor. Examination periods are determined by
legislative requirements however, are commonly performed on a six
or twelve monthly periodic cycle
Nominal Test A test load equal to the WLL that is applied to an item of lifting
equipment to prove its integrity up to the maximum WLL
Static Lift The lifting of an item that due to a particular service condition does
not amplify the weight of the item such as lifting cargo from a road
trailer
Dynamic Lift The lifting of an item that due to a particular service condition
amplifies the weight of the item such as lifting cargo from a supply
vessel
Categorisation A level of complexity in performing the lifting task considering other
risk factors such as weight or cost
In-service Inspection A visual inspection and/or function test of an item of lifting equipment
usually performed prior to each use.
Colour Code A simple determination of the examination validity of a particular item
of lifting accessory or lifting equipment to be used
Competent Person A person who has sufficient experience, training and knowledge in
performing a specific trade such as riggers or crane operators
Lifting Equipment An item of lifting equipment that can physically lift such as cranes or
manual chain blocks
Lifting Accessory An item of lifting equipment that can not physically lift on its own such
as slings or shackles

results of a categorisation assessment by a competent person. Note this lift is a heavy lift.

2.4 PROCEDURES FOR REJECTING CONSIGNMENT OR DELIVERY OF NON-


CONFORMING LOADS

All offshore containers or units shall be inspected and certified in accordance with DNV GL
– CG – 0051. This procedure established shall be approved by Level 3 qualified personnel
in terms of the testing method. Non- Destructive examination shall be in accordance with
DNV GL – ST – E271. The reports issued at least containing the following information:
a) NDT method and procedure used
b) NDT parameters necessary for a proper assessment
c) Acceptance standards used
d) Confirmation of acceptance and rejection
e) NDT operator level of qualification

2.5 SCOPE OF RISK ASSESSMENT CARRIED OUT

This shall be done in conjunction with the Hazard Risk Identification and Risk
Assessment (HIRARC) as performed in accordance with project requirements and
comply with PCSB PTS requirements as well as the risk matrix (RAM).

All identified hazards have been controlled and mitigated to ALARP (As Low As
Reasonably Practicable). Refer to Appendix B

4.0 LIFTING MANAGEMENT

4.1 WEATHER FORECAST

The Crane Operator have right to exercise a ‘Stop Work’ or ceased the operation when
he feels to do so based on his professional judgement:

a. Sudden increase of wind speed or strong gust of wind


b. Heavy Rain associate with thunderstorm.

4.2 RESPONSIBILITIES

P & A OIM

Has overall responsibility for the lifting operation under his area of authority. Has responsibility for
the authorization of this generic LOP and that the practices applied by the lifting team are in
compliance with regulatory and PCSB company and golden rules.

He shall be responsible for the implementation of the lifting Operation Plan at his site and shall agree
for the operation to commence. He shall satisfy himself of the competence of all the crane operators,
riggers, and crew involved before granting approval. He shall be cognizant of all-weather and
environmental constraints as defined within this LOP when considering approval to proceed. He
shall ensure that appropriate contingency precautions are in place.

Lifting Supervisor

Ensure the safety of the lifting operation by applying this generic LOP as a method of performing
the lifting task. Will appoint and ensure that personnel are is competent to perform this task safely.
Has the responsibility to ensure that the lifting team on board the vessel has available a sufficient
RA and LOP and that a pre-lift toolbox checklist has been performed, that will result in the lifting
operation being performed as safely as possible. Ensuring that the personnel performing the lifting
operation are competent to do so and that all control actions to reduce risk are identified.

WELL SITE SUPERVISOR

Ensure the safety of the lifting operation by applying this generic LOP as a method of performing
the lifting task. Will appoint and ensure that personnel are competent to perform this task safely.
Has the responsibility to ensure that the lifting team have available a sufficient RA and LOP and that
a pre lift toolbox checklist has been performed, that will result in the lifting operation being performed
as safe as possible. Ensuring that the personnel performing the lifting operation are competent to
do so and that all control actions to reduce risk are identified.

WORK PERMIT APPLICANT

Ensure that a specific LOP and RA have been written and that the review of these documents is in
date and valid prior to issue of a Permit to Work. This responsibility to review the JHA and work
permit falls to HSE Advisor.

MATERIAL CO-ORDINATOR (MATCO)

Responsible to ensure the preparation of the cargo manifest and the computation of all lifted weights
in consideration of the SWL of cargo container units, slings and lifting equipment.

He shall communicate all manifested weights with the crane operator and roustabout to ensure that
the crane SWL is not exceeded.

He shall ensure that all offshore unit check lists have been received and reviewed and that all
returning offshore unit check lists have been completed and communicated to the shore base.

He shall ensure that the cargo manifest is accurate and communicated to the Lifting Supervisor and
Crane Operators.

CRANE OPERATOR

Shall ensure that all pre-use examination of the crane has been completed, and there are no defects
to the crane that may affect the crane and the safety of the lifting task being performed. He shall
ensure that the crane configuration, crane radius or boom angle is correct for the load to be applied.

CRANE SIGNALMAN / BANKSMAN

He shall perform the pre-use checks of the lifting attachments such as shackles, slings and cargo
container units and shall complete the returning Offshore Unit Check List. He shall be responsible
for following this LOP method as the process for the lifting task to be performed. Where deviation
from this procedure has required a review of the specific risk assessment and this LOP shall be
performed

He shall act as the dedicated Banksman for the crane operator and follow the Banksman duties and
responsibilities during the lifting processes.

5 METHOD (WORK SEQUENCE)

5.0 SITE PREPERATION

The crane operator and roustabout shall review a risk

The TOS supervisor (permit co-coordinator) shall issue a Permit


4 to Work only where supporting document is submitted TOS Supervisor

The crane operator shall perform the Pre-Lift Tool Box Talk and
5 complete the appropriate toolbox talk checklist ensuring all
personnel are conversant with the permit to work, the RA ad the Crane Operator
LOP.

The crane helper shall erect signs and barriers and perform a PA
6 announcement that cargo handling operations are to commence.
TOS Crew & Crane Helper
Only those persons authorized to, trained and competent to work
with the crane shall enter the work area.
The crane operator: shall perform the pre-use checks of the
7 crane in accordance with manufacturer instructions and TOSSB Crane Operator
requirements. He shall record on to the appropriate pre-use
check list format. He shall confirm that the weather and operating
constraints are not exceeded for the particular crane and lifting
task to be performed.

He shall ensure that there is sufficient deck area available to land


8 the loads being offloaded and that the load bearing capacity of
Crane Helper
deck structure is sufficient for the intended loads.

5.1 PERFORM THE LIFT

Task Description Performed by


The crane operator shall attach a suitable cargo handling
1 pennant with suitable length and sufficient WLL to the crane Crane Operator
hook. The pennant hook shall be of the closed safety type and
under only special circumstances i.e. non routine lifting shall an
open jaw hook with safety latch be utilised for handling cargo to
and from supply vessels.

The banksman shall direct the crane operator using radio or BS


2 7121 hand signal methods (refer appendix A) to position the Banksman
pennant safety hook to a position directly above the CoG of the
load, container or basket to be hoisted. The crane hook shall be
at a position slightly before the CoG to allow for deflection of the
crane boom due to a heavy lift.

The crane operator shall follow the banksman directions at all


3 times unless he can see a consequence where the safety of the Crane Operator
lifting task may be jeopardized. All crane motions shall be
performed in a controlled and steady manner. Where blind lifting
is being performed the crane operator shall stop all motions if a
repeat signal is not periodically given by the banksman usually
every fifteen seconds.
The crane helper shall attach the lifting assembly master link to
4 the crane safety hook. Ensure that there are no twists to the
assembly and that all shackles are secure and are fitted with Crane Helper
safety pins

The roustabout shall inspect the engine lifting arrangement


5 before lifting ensuring that no foreign matter can fall from the Roustabout
carrier and that the lifting arrangement is secure

The roustabout may attach tag lines of sufficient length to the Roustabout
6 appropriate tag line attachment point if required.

Once all personnel are clear of the load and in a safe area the
7 banksman may instruct the crane operator to slowly tension the Banksman
lifting assembly

The crane operator and roustabout will ensure that the hoisted
8 load does not exceed the capacity of the lifting arrangement nor Crane operator
the cargo handling pennant nor the crane SWL capacity Roustabout

The banksman shall instruct the crane operator to continue to


9 hoist the load clear of the area and to utilize crane motions to Banksman
transfer the load to the settling area.

The roustabout should stand clear of the load being hoisted. If


10 required he may utilize the attached tag lines or a cargo handling Roustabout

push pull tool to orientate the load. At no time will cargo that is
suspended on the crane be directly handled by placement of
hands. No Hands on Loads .

The crane operator will move the load and shall maintain the load
11 at a suitable height to enable the tag lines if attached to travel Crane operator
freely. He shall not transport a load over the top of personnel and
shall sound the warning siren if required

The crane operator shall slew the load at the settling area as
12 much as possible and under the direction of the Banksman. Crane operator

The roustabout shall remain in a safe location until the unit has
13 been landed in the desired position or close to position. If Banksman

5.5 CONTINGENCY

In the event of an emergency, the lifting team shall complete the immediate lifting
sequence and shall ensure that the crane / load is left in a safe position before continue
to respond.

In the event of equipment failure or breakdown, shut down all powered lifting
equipment and ensure all braking systems are applied. The Crane supervisor shall be
immediately informed and a technician shall attend the equipment to determine a
solution or repair before re-start of any powered systems

In the event of an unplanned occurrence, if safe to do so, secure the load and
barricade the area. The Crane supervisor and HSE Advisor / Wells Site Leader shall
be immediately informed and shall determine a solution or load recovery method.
