1.2.8 Use appropriate PPEs like face mask and gloves.
1.2.9 Checklist for COVID-19 Symptoms for each employee were being
taken.
1.2.10 All employees found with related symptoms were immediately,
reported to Site Male Nurse or Camp Clinic for immediate isolation
and further medical management.
2 Cutting, grinding and welding of 2.1 Fire 2.1.1 Ensure that lines and equipment containing or have a potential of CAT Engineer /

2.5 Personnel getting injured 2.5.1 When “cracking” the cylinder valve (i.e., clearing the valve of dust CAT Engineer /
from the compressed or dirt prior to connecting the regulator), personnel shall stand to Supervisor /
gas cylinder one side of the valve/regulator outlet, not in front of it. Foreman /

2.6 Electrocution from the 2.6.1 Generator shall be grounded before use. CAT Engineer /

2.6.4 Direct current (DC) shall be used for welding operations in any
situation when the effect of electric shock is likely to be extreme.
2.6.5 Manual electrode holders shall be specifically designed for arc
welding and have a capacity capable of handling the maximum
rated current required by the electrodes.
2.6.6 Any current-carrying parts of the electrode holder which a welder
has in his hand (and the outer surfaces of the jaws of the electrode
holder) shall be fully insulated to protect against the maximum
voltage to ground.
2.6.7 Electrode holders shall be constructed to accommodate all sizes
of electrodes.
2.6.8 The frames of all electrical arc welding machines shall be properly
grounded.
2.6.9 Ground returns shall be securely attached by cable lugs, clamps
or bolts to the material being welded upon.
2.6.10 Ground returns shall be connected as close as possible to the
location being welded upon to avoid stray currents.
2.6.11 When the structure or nonhazardous piping being welded is used
as a ground return circuit, it shall be ensured that electrical
conductivity to ground exists in the structure or piping.
2.6.12 Splices are not allowed anywhere in welding cables.
2.6.13 Welding cables shall be continuous for a minimum distance of 3 m
(10 ft) from the end to which the electrode holder is connected.
2.6.14 If a welding cable becomes damaged beyond 3 m (10 ft) from the
end to which the electrode holder is connected, it shall be repaired

2.12 Hot weather 2.12.1 Schedule the most strenuous work during the coolest times of the CAT Engineer /

2.12.2 Minimize work time in direct sunlight as much as possible. Foreman /
2.12.3 Employees shall not be allowed to work alone. Mechanical crew /
2.12.4 Supplies of cool drinking water shall be made available for HSE Officer / WPR
workers.
2.12.5 Display educational information related to heat stress conditions,
illness/symptoms and preventive measures on bulletin boards and
break/recovery areas.
2.12.6 Provide drinking water stations (e.g., coolers with chilled or ice
water) for workers and remind them to drink plenty of water even if
not thirsty.
2.12.7 Monitor the heat-related health condition of employees, as well as
the effectiveness of any engineering/administrative controls and
personal protective equipment (PPE) being used.
2.12.8 Recognize early signs and symptoms of heat-related illness and
take appropriate action to prevent serious heat illness
2.12.9 Take appropriate action upon developing symptoms of heat
cramps, heat exhaustion or heat stroke.
2.12.10 Wear appropriate clothing (e.g., light colored, lightweight,
breathable, loose clothing is best).
2.12.11 Any employee experiencing symptoms of a heat-related illness
shall immediately stop work and report to his supervisor.
2.12.12 The victim shall be allowed to fully recover in an appropriate
recovery area before returning to work.
2.12.13 Victims of severe heat exhaustion or heat stroke shall be
immediately evacuated to the nearest medical facility with a
qualified medical doctor who can provide proper treatment. Such
incidents shall be properly reported.

3 Lifting and installation of pipe 3.1 H2S 3.1.1 Gas test shall be conducted and monitored using a calibrated gas CAT Engineer /
using: detector (e.g. MX4). Supervisor /
3.1.2 Personal H2S detector (e.g. T40) shall be provided for the Foreman /
Crane workers. Mechanical crew /
Hand tools 3.1.3 Self-contained breathing apparatus shall be available. HSE Officer / WPR
3.1.4 All ignition source shall be removed. Crane shall not be started
unless it is safe to do so.
3.1.5 Wind direction shall be monitored. Workers shall be positioned
upwind from the H2S source.
3.2 Fire 3.2.1 LEL shall be monitored using a calibrated gas detector (e.g. MX4). CAT Engineer /
3.2.2 Brass hammer shall be used to prevent sparks. Supervisor /
3.2.3 Fire extinguisher shall be available. Foreman /
3.2.4 Flame resistant clothing shall be worn by all workers. Mechanical crew /
3.2.5 Crane shall not be started unless safe to do so. HSE Officer / WPR
3.3 Personnel getting struck 3.3.1 Crane operators shall be properly licensed and certified by SA for CAT Engineer /
by equipment the crane type/model they are using. Supervisor /
3.3.2 Crane lift areas shall be barricaded, and warning signs shall be Foreman / Operator
posted. / HSE Officer /
3.3.3 Only authorized personnel shall be permitted inside the barricaded WPR
area.
3.3.4 Back-up alarm shall be installed for the equipment.
3.3.5 Spotter or flagman shall be assigned for the equipment while
maneuvering.
3.4 Equipment failure 3.4.1 Crane shall have a valid inspection sticker issued by SA or by an CAT Engineer /
approved 3rd party inspection agency. Supervisor / Foreman
3.4.2 Pre-inspection of equipment shall be conducted by operator prior / Crane operator /

3.5 Tipping of crane 3.5.1 The load shall be within the safe working load / capacity of the CAT Engineer /

3.6 Falling of load while 3.6.1 Crane lifts shall not be performed in wind speeds exceeding 32 CAT Engineer /

3.6.2 The crane operator shall not leave the controls at any time while a HSE Officer / WPR

3.8 Rigging tackles failure 3.8.1 All riggers shall be certified by SA. The rigger shall have CAT Engineer /
responsibility for coordinating activities of all personnel involved Supervisor /
with the lift. Foreman / Rigger /
3.8.2 Rigging tackles shall be inspected by the certified rigger prior to HSE Officer / WPR
each lift.
3.8.3 Fasteners or other rigging hardware manufactured in-house from
bolts, rods, pins, hooks, etc., shall not be used.
3.8.4 Slings shall not be modified, repaired, shortened, or lengthened.
3.8.5 Defective or damaged slings and other rigging hardware, or slings
and other rigging hardware not complying with SA requirements,
shall be recorded in the Sling Inspection Log, and immediately
removed from the job site and destroyed.
3.8.6 Working conditions that may affect slings and other rigging
hardware shall be evaluated, with special attention to
temperatures, chemicals, abrasions, etc.
3.8.7 Slings shall be rigged to provide the best load control.
3.8.8 The force on each sling during a lift shall be less than the sling
manufacturer’s rated SWL.
3.8.9 Other rigging hardware (e.g., shackles, eyebolts, hooks, rings, and
links, coupling pins) shall be selected to provide capacities equal
to or greater than the SWL of the sling.
3.8.10 Slings and other rigging hardware shall be stored to protect them
from mechanical damage, corrosives, direct sunlight, moisture,
extreme heat, etc.
3.8.11 All crane lifts shall be partially lifted (e.g., floated for at least 3
seconds) to ensure rigging and lift integrity prior to proceeding with
the actual lift.
3.9 Defective Tools 3.9.1 Ensure equipment and tools (hand tools are in good operating CAT Engineer /
condition and properly used. Supervisor /
3.9.2 Ensure tools constructed of good quality materials are used. Use Foreman /
of “homemade” tools is prohibited. Mechanical crew /
3.9.3 Ensure equipment defects are promptly repaired and defective HSE Officer / WPR
equipment is immediately ―red tagged, removed from service,
and discarded/destroyed.
3.9.4 Properly use the correct tools and equipment for the job.
3.9.5 Keep tools in good condition and repair.
3.10 Poor ergonomics 3.10.1 Workers shall be instructed in proper manual material handling CAT Engineer /
techniques and practices prior to their work assignment. Supervisor /
