1 Obtain work permit 1.1 Lack of hazard 1.1.1 The issuer shall approve and issue a permit only after conducting a CAT HSE / SA
awareness joint site inspection with the receiver to identify potential hazards, Issuer
perform necessary gas tests, and write on the work permit the
specific precautions necessary so the work can be performed safely.
1.2 COVID-19 infection and 1.2.1 Temperature screening shall be conducted for all employees before CAT Mngmt. /

1.2.6 Employees were encouraged to avoid touching their eyes, nose, and
mouth.
1.2.7 Practicing respiratory hygiene by coughing or sneezing into a bent
elbow or tissue and then immediately disposing of the tissue.
1.2.8 Use appropriate PPEs like face mask and gloves.
1.2.9 Checklist for COVID-19 Symptoms for each employees were being
taken.
1.2.10 All employees found with related symptoms were immediately,
reported to Site Male Nurse or Camp Clinic for immediate isolation
and further medical management.
2 Positioning the Low Bed Trailer 2.1 Equipment failure 2.1.1 Equipment shall be inspected periodically by a competent equipment CAT Engineer /
inspector or mechanic. Supervisor /
2.1.2 Pre-use inspections shall be conducted on all equipment. Operators Foreman / Driver
performing these inspections shall ensure the following, at a / Mechanic /
minimum, prior to operating the equipment: HSE Officer /
No fuel, oil, hydraulic fluid, water, etc., leaks are coming from the WPR
equipment. Spills shall be immediately contained, cleaned up,
and properly disposed.
Hose and pipe connections are checked and free of wear and
cracks. Defective hoses/pipes shall be replaced or repaired. All

2.4 Overhead powerlines 2.4.1 Overhead power lines shall be protected by barriers and/or CAT Engineer /
timber/wire “goal posts” to maintain required clearances. Supervisor /
2.4.2 Access below overhead power lines shall be restricted by barriers Foreman / Driver
where the “goal posts” are located. / Spotter / HSE
2.4.3 Low bed shall not be positioned directly below overhead powerlines. Officer / WPR
2.4.4 When heavy equipment is operating near an energized power line, a
designated signalman shall be assigned to ensure the following
minimum distances are maintained:

2.5 H2S 2.5.1 Gas test shall be conducted and monitored using a calibrated gas CAT Engineer /
detector (e.g. MX4). Supervisor /
2.5.2 Personal H2S detector (e.g. T40) shall be provided for the workers. Foreman / Driver
2.5.3 Self-contained breathing apparatus shall be available. / Spotter / HSE
2.5.4 Wind direction shall be monitored. Low bed shall be positioned Officer / WPR
upwind from the H2S source.
2.6 Fire from combustible 2.6.1 LEL shall be monitored using a calibrated gas detector (e.g. MX4). CAT Engineer /
gases 2.6.2 Fire extinguisher shall be available. Supervisor /
2.6.3 Flame resistant clothing shall be worn by all workers. Foreman / Driver
2.6.4 Equipment shall not be started unless safe to do so. / Spotter / HSE

3 Opening of ramp 3.1 Ramp malfunction 3.1.1 Always inspect the ramps to ensure they function properly. CAT Engineer /

3.2 Manual handling 3.2.1 Ramps must be provided with spring loaded jacks in order for a CAT Engineer /
single person to easily and safely open ramps. Supervisor /
3.2.2 The assignment shall consider personal physical limitations that vary Foreman / Driver /
among individuals. Care shall be taken so as to not exceed these HSE Officer /

limitations.
3.3 Personnel getting struck 3.3.1 Workers shall not be allowed to stand near or directly behind the CAT Engineer /

Crane 4.1.2 Pre-use inspections shall be conducted on all equipment. Operators Foreman /

4.2 Personnel getting struck 4.2.1 Operators and drivers shall possess a valid Saudi Arabian CAT Engineer /
by equipment government (SAG) heavy equipment license and SA certification as Supervisor /
required for operation of the specific type of heavy equipment. Foreman /
4.2.2 Equipment shall be installed with back-up alarm to alert the Operator /
personnel. Spotter / HSE
4.2.3 Spotter and/or flagman should be assigned with whistle, reflective Officer / WPR
vest, green and red colored flags or reflective go and stop signs to
separate the personnel from equipment.
4.2.4 Spotter and/or flagman should not stay in the blind spot of the
equipment and be always visible to the operator.
4.2.5 Spotter and/or flagman should not walk in line with the path of the
equipment.
4.2.6 Construction equipment shall have both TB PWAS and RVCCCS
installed, tested, and operating at all times.
4.2.7 Personnel and visitors shall be provided by CONTRACTOR with
Tags compatible with the TB PWAS system and shall be required to
wear them at all times.
4.2.8 Personnel shall not rest or sleep (e.g., seek shade) under mechanical
or heavy equipment at any time.
4.2.9 Loading & unloading area shall be barricaded and isolated to prevent
the entry of unauthorized personnel.
4.3 Falling of equipment 4.3.1 Always park the trailer on a solid level surface that can easily support CAT Engineer /
the load. Supervisor /
4.3.2 Operators shall move the equipment slowly while being loaded to or Foreman /
unloaded from the low bed. Operator /
4.3.3 Spotter shall be assigned to guide the operators during loading or Spotter / HSE
