Description of the Incident:
English: On March 25th at 00:30 AM, a BKE welder was lighting the oxy-acetylene torch in order
to cut a sheet. A flashback occurred causing flames on the pressure gauge side of the acetylene
bottle causing the pressure gauge burned even gas hoses to melt. A manual fire extinguisher was
used to control the fire and the gas bottle valves were immediately closed, thus controlling the
acetylene leak. No injuries were recorded
Français: Le 25 mars vers 00:30, un soudeur de BKE était en train d’allumer le chalumeau
oxyacétylénique afin de découper une tôle. Un retour de flamme s’est produit entrainant les
flammes du côté du manomètre de la bouteille d’acétylène causant la calcination du manomètre
et la fonte des boyaux de gaz. Un extincteur a été utilisé pour maitriser le feu et les vannes des
bouteilles de gaz ont été immédiatement fermées, maitrisant ainsi la fuite d’acétylène. Aucune
blessure n’a été enregistrée
