MOTION FOR EXECUTION

Complainant through and by himself to this Honorable Office most

respectfully manifest:

1. That a Decision dated November 27, 20202 was rendered by this

Honorable Office in the above-entitled case, the dispositive portion
of which is quoted as follows:

“WHEREFORE, premises considered, judgment is hereby

rendered ordering respondent _______________ to pay complainant
his proportionate 13th month pay and service incentive leave pay
in the amount of PhP_______________.”

SO ORDERED”

2. That no appeal was filed within the reglementary period pursuant

to the Rules of the Commission, making the said Decision final and
executory. Attached is the Certificate of Finality issued by the
Honorable Office dated _______________;

3. That up to the writing of this motion, respondents in the above-

captioned case have not yet complied with its obligations arising
from the said Judgment/Decision.
 PRAYER

WHEREFORE, in the light of the foregoing it is most respectfully prayed

of this Honorable Office that a writ of execution be issued to order the
respondents to pay complainant the amount of PhP_______________ as stated
in the Decision of the Honorable Office.

Other relief and remedies just and equitable under the premises are
likewise prayed for.
