SUPPLEMENTAL AFFIDAVIT

I, JESSICA V. FLORES, of legal age, Filipino, singe and a resident of

Brgy. Catbangen, San Fernando City, La Union, after having been duly sworn
to in accordance with law hereby depose and state:

1. Last ___________, I went to Producer’s Bank La Union Branch to seek my

employment papers because I was already hired at a company in
Manila;
2. I was Bank Teller at Producer’s Bank La Union Branch and left said Bank
for greener pasteur last _________________;
3. At said date, I was confronted by the Area Head of Producer’s Bank in
the person of Mr. Marvin Madrigal regarding an anomaly involving
Maricel Bagio, the bank’s cashier then;
4. I denied the allegations of my involvement in the anomaly. They
confronted Ms. Maricel Bagio who in turn executed two (2) letters
dated November 11, 2017 for Ms. Filipinas Fernandez, the EVP of the
aforementioned bank;
5. The two letters were executed before Mr. Marvin Madrigal and Ms.
Lorebel Tadina, bank officers;
6. In the said letters, Ms. Maricel Bagio narrated how the anomaly
transpired and freed me from any participation or involvement of
whatever kind in the said anomaly involving deposit accounts of clients
of the bank;
7. The first letter was used by the bank against me while I also used the
second one in my defense which was attached as Annex “2” in my
counter-affidavit and my motion for reconsideration;
8. The said letter (Annex 2) was only a photocopy because the original
was crumpled by Mr. Marvin Madrigal due to his anger when she found
out that it was all done by Maricel Bagio;
9. Before said crumpling, I was given a photocopy of the said letter to
which Mr. Madrigal and Ms. Tadina acknowledged because of them
signing said letter noting that said letter is a faithful reproduction of
the one executed by Maricel Bagio;
10. I am executing this affidavit to attest to the truth of the foregoing
and to further support my motion for reconsideration of the Qualified
Theft filed against me by the bank;
