2022: SCHOOL SAFETY ASSESSMENT TOOL (SSAT) 2022 FOR THE PROGRESSIVE
EXPANSION OF THE FACE-TO-FACE LEARNING MODALITY
FOR THE SCHOOLYEAR 2022-2023

This Memorandum of Agreement made and executed between:

BARANGAY MOLINO II, with postal address at Progressive Village 4, Barangay Molino
II, City of Bacoor, Cavite, represented herein by MICHAEL J. SAQUITAN, PUNONG BARANGAY
of Barangay Molino II, City of Bacoor, Cavite, hereinafter referred to as the BARANGAY, the First
party,
and
ST. ALPHONSUS LIGUORI INTEGRATED SCHOOL, with postal address at Addas 2
Village, Molino II, Bacoor City, Cavite, represented herein by Ma’am CECILIA C. SIPIN, SCHOOL
DIRECTOR, hereinafter referred to as the Second Party
Witnesseth:
The two parties hereby bind themselves to undertake a MEMORANDUM OF AGREEMENT for
the purpose of supporting the Department of Education in the compliance of the SCHOOL
SAFETY ASSESSMENT TOOL (SSAT) 2022 FOR THE PROGRESSIVE EXPANSION OF THE
BLENDED LEARNING MODALITY (COMBINATION OF FACE-TO-FACE AND ONLINE) FOR
THE SCHOOL YEAR 2022-2023.

The SSAT is not the final determinant of the school’s participation in the progressive expansion
of face-to-face classes. It is only meant to prepare schools for the eventual reopening, inform
them of the required indicators and standards that they need to meet to ensure safety of the
learners and school personnel. This tool shall also serve as guide to the Schools Division Office,
Regional Office and Central Office in providing support and technical assistance to the schools.

A. AREAS OF COLLABORATION:
Both parties will respect what is agreed upon during the meeting done in June 24, 2022
at the Session Hall of the Sangguniang Barangay, regarding the SCHOOL SAFETY
ASSESSMENT TOOL (SSAT) 2022 FOR THE PROGRESSIVE EXPANSION OF THE
FACE-TO-FACE LEARNING MODALITY, in the presence of the following:

C. SECOND PARTY REPRESENTATIVE is Ma’am CECILIA C. SIPIN, School Director of
St. Alphonsus Liguori Integrated School

D. The following are RESPONSIBILITIES of the FIRST PARTY:

1. Barangay Molino II, in coordination with the City Health Office and Medical Office of
the City Schools Division, shall task the Barangay Health Emergency Response
Team (BHERT) and Barangay Health Unit (BHU) to check the school’s compliance
with the Revised School Safety Assessment Tool (SSAT) before a monitoring team
from the School Division Office visits them.

2. Barangay Molino II offers SALIS venue, transportation services and, Barangay Health
Workers’ (BHW) services and Bantay Bayan’s assistance for safety on any school-
related activities and projects in connection with its compliance to the SCHOOL
SAFETY ASSESSMENT TOOL (SSAT) 2022 FOR THE PROGRESSIVE
EXPANSION OF THE FACE-TO-FACE LEARNING MODALITY.

D. The following are RESPONSIBILITIES of the SECOND PARTY:

1. St. Alphonsus Liguori Integrated School, upon having passed the stringent
requirements based on their respective School Safety Assessment Tool must
establish their triage right at the entrance of the school compound which should be
complete with alcohol and face masks for the use of the students, teachers and
non-teaching personnel;

2. All individuals who will be entering the compound of St. Alphonsus Liguori
Integrated School are mandated to wear their face masks at all times.

3. If there will be children who will be entering the school premises without face
masks because they might have left the same or might have lost it on the way, they
will be provided at the triage area to allow them to enter the school and attend their
class schedules.

4. If there will be confirmed COVID-19 case/s among teachers and/or other school
personnel and students during the course of the limited face-to-face classes,
appropriate contact tracing will be done to ascertain the extent of infection followed
by the implementation of granular lockdown in the classroom where the case/s
were discovered.

5. Classrooms where the COVID-19 case/s has/have been found will be disinfected
before face-to-face classes will resume.

6. Students, especially the older ones, should be constantly reminded to directly go

home after their class schedules to prevent them from being exposed to the deadly
virus.
