AFFIDAVIT OF LOSS

I, JAYSON TUAZON SORIAO, of legal age, Filipino, single, and with postal and
residence address at H-2 Dasmarinas City, Cavite, after having been duly sworn to in accordance
with law hereby depose and say that:

That I was a student of Dasmarinas North National High School where they issued me
a SCHOOL DIPLOMA when I graduated on 2014-2015;

That the said SCHOOL DIPLOMA was lost and can no longer be found, possibly due to
it being misplaced or unknowingly in our home;

That despite of diligent efforts to find and locate the said document, all efforts are proved
in vain;

That I am executing this affidavit to attest to the truth of the foregoing and for all legal
intents and purposes.

IN WITNESS WHEREOF, I have hereinto set my hand this 15 th day of July 2022 at
