• Prior to completing this form you are to review the Academy Core Values, and the PMA Philosophy of
Education. (see attached)
• You are to complete this form over a 72-h period and then make any amendments after a 24 h period has
elapsed.
• This form is only valid for developmental and evaluative purposes if approved by the trainee’s DO.
• Your goals should be achievable in a semester- if you have a longer-term goal this can be included on a
case-by-case basis but progress should be measurable at the end of the first period.
• This is only a format. This is only a guide. You can change this format as you wish and add more goals.
