Staff observed that resident has lot of pain in legs
while giving care.
Plan- Baclofen 10mg BID.
Tylenol 500mg BID
Pt C/O generalized joint pain
Plan- Butrans patch 5mg -1patch X1 week.
Hydromorphone 3mg PO Q3h PRN
Refer to Dr. Milne done.
Redness and bleeding on tongue X 1 week
Plan- Apply glycerine topically to tongue BID.
Lab reviewed.
High Phenytoin levels and UTI.
Plan- Phenytoin 150mg PO OD (Dose deceased).
Macrobid 100mg BID X 10 days
Check phenytoin level next week.
Ano – rectal region red, inflamed.
Plan- Proctosedyl oint topical application BID
Anusol- HC suppository 1per day X 2 week.
Will reassess as needed.
Lab results reviewed.
Referral sent to Dr Wojick W.
Ear examination done – syringing next week.
Perineal region redness.
Plan- D/C Hydroval
Apply Fucidin H cream and zinc Oxide affected
region.
Keep area clean.
Lab reviewed- UTI.
Plan- Macrobid 100mg PO BID X 10 days.
Drink water + diluted cranberry juice.
Pt having redness and scrotal rash.
Recurrent UTI.
Plan- Start with ciprofloxacin 500mg PO BID X 7
days.
Apply lortiderm cream topically application

Plan- Monistat ovules intravaginally OD x 3 days.

Plan- Mobilize lower legs – Do exercise.

B/L Pitting oedema.
Furosemide 60mg AM and 20mg Noon.
 Elevate legs on 2-3 pillows.
Wear compression stocking.
Will reassess again as needed.

Seroquel 25mg HS, donepezil.
Seroquel 50mg PO BID.
Seroquel 12.5 mg PO BID PRN.
