II. NARRATIVE REPORT/EXECUTIVE SUMMARY

Caniogan High School held a faculty meeting on February 15, 2022 at Caniogan High School covered
court. The meeting was held to relay the results of the re-evaluation of school facilities in preparation of
progressive expansion in mass limited face-to-face classes and to identify the points that the school still lacks. The
meeting started with an opening prayer led by Mrs. Jezza G. Lapiguera.

Mrs. Zenaida facilitated the meeting and echoed the requirements needed by the school. Such requirements
are listed as follows:

1. Each KRA should obtain a folder. 


2. Update the folder for parent’s consent per class- upload the new consents to the designated Google Drive
(as of today, 68% of parents, consented to the face-to-face classes for SHS learners) 
3. Sir Pao will make a template for every document needed. 
4. Size of document for lamination: A4 
5. Size of document for filing: Long/ 8.5 “x 13” 
6. Orientation of all pictures: landscape

7. Pictures of the following are needed: 


 every electric fan in each room 
 windows (before cleaning, during cleaning, after cleaning) 
 comfort room inside the canteen as well the hand washing area 
 Health and safety kit (one close up and one long shot) 
 Thermal Scanner 
 Alcohol Dispenser 
 Four trash bins and one solo picture of yellow trash bin 
 School nurse- Ma’am Reyna 
 Clinic Teacher- Sir Erwin 
 PPE’s held by the advisers 
 Health and safety signages 
 Boxes of contract tracing forms (per room) 
8. Comfort rooms 
 Girl students of Grade 11- ABM and Grade 11&12- HUMSS: CR in Grade 9- Diamond. 
 Girl students of Grade 11 & 12 HE, Grade 12- ABM and Grade 12 GAS will proceed to the
canteen. 
 Girl students of Grade 11 & 12 ICT, and Grade 11- GAS will proceed to the clinic  
 Boy students will go to the comfort rooms located beside Grade 11-ABM room 
 Water supply is not yet available 
 Students will proceed to Grade 9- Silver for water source. 
9. All documents for door posting should be laminated. 
10. Obtain SHS teaching loads from Ma’am Zeny. 
11.  Inventory of modules: submit to Sir Pao (3rd and 4th Grading modules’ ratio should be 1:1). 
12. The students reflected on the class list should be the students who are allowed for face-to-face classes and
the students who are NOT allowed should be noted below the document/file. 
13. The correct sample template (including the font style and size, spacing, and the page size) of class list will
be sent by Ma’am Eunice. 
14. Data/Reports of the following are needed: 
 Profile of learners’ health status (by adviser): data c/o Ma’am Eunice then template c/o Sir Pao 
 Record of community vaccination status from Municipal Health Officer 
15. Clinic sink curtain- c/o Sir Erwin 
16. Isolation room (rebuilding)

Indeed, the event was a success. All teachers agreed with the tasks and deadlines.
