AFFIDAVIT OF TWO DISINTERESTED PERSONS

WE, RUBEN P. ALDAY and BILL BOY G. VERGARA, both of legal ages,
Filipinos, with residence and postal address at Brgy. Pilit, Sta. Fe, Leyte, after
having been sworn to in accordance with law hereby depose and state the
following, THAT:

1. WE are friends of the SUMAYOD FAMILY of Brgy. Pilit,

Sta. Fe, Leyte having been our neighbors and we had
always been present in social gatherings and other
communal affairs. As such we are familiar and know
some important events of their lives, in particular we
know the SPS. ALVIN and ANGEL SUMAYOD;

2. We likewise know that Angel, gave birth to XANDRINA

FAYE INOCANDO SUMAYOD on SEPTEMBER 15, 2010
at Palo, Leyte;

3. Such birth of XANDRINA FAYE was not

recorded/registered with the Local Civil Registrar of Palo
due to unawareness of civil registration laws, rules and
regulation;

4. WE are executing this affidavit to attest the truth of

the foregoing information and for the late registration of
XANDRINA FAYE INOCANDO SUMAYOD with the Office
of the Local Civil Registrar, Palo, Leyte;

IN WITNESS WHEREOF, we have hereunto set my hand this 29 th day if
