of Brgy. Arado, Palo, Leyte do HEREBY, AUTHORIZES GERLOU C. ORAYA, likewise

1. TRANSACT with Philippine Statistics Authority (PSA) pertaining to

the my application to get a copy of my Certificate of Live Birth;

2. SECURE/GET copy of the said Certificate of Live Birth in my behalf;

3. EXECUTE and SIGN any documentary requirements in my behalf for

the purposes and pursuant to the authorities above mentioned.
