AFFIDAVIT OF UNDERTAKING

of legal age, presently assigned with Municipal Police Station, designated as

Officer-In-Charge, after having been duly sworn to, under oath and in accordance
with law, do hereby depose and state that:

1. That at around 6:11 on the evening of July 22, 2022, received a

memorandum letter from with subject Submission of Affidavit of
Undertaking dated July 21, 2022.

2. The memorandum pertaining to the directive from higher headquarters re

Reminder on the Utilization of e-Subpoena System and further reminding
that violation of the said directives constitutes an offense of Less Grave
Neglect of Duty, defined under NAPOLCOM Memorandum Circular No.
2016-002, Rule 21, Section 2(B), paragraph 1(a).

3. In connection above, the undersigned have read, fully understood and

properly disseminated the said directive to all personnel of Tabango
Municipal Police Station.

4. That I am executing this affidavit in compliance with the said memorandum

from C, RIDMD dated July 21, 2022.
