Tisa Cebu City, Philippines, after having duly sworn to in accordance with law, depose and
say that:

1. I am the owner of the business name Julmer Food Ventures with Permit No. 90155B;

2. That the above-mentioned business has its principal address at Katipunan St., Tisa
Cebu City;

3. From 2019 up to present, the above-mentioned business has stopped its operation
due to some personal reason/issues with the lessor;

4. This affidavit is executed for the purpose of retiring of the busines permit from the
City of Cebu;
