Assessment of Older Adults Assignment - Spring 2020

NOTE: This assignment replaces the Client Assessment assignment designated at the beginning of the
semester. Since the COVID-19 pandemic halted clinical experiences, it is not possible for students to
complete a client assessment. This assignment alternatively demonstrates the student’s ability to
understand and use common assessment tools appropriate for older adults. Total points for this
assignment have been reduced from 70 to 35.

PART 1: Scoring Assessments

PART 2: Assessment Completion (Minimum Data Set – Section F)


Identify someone who is willing to role play an older adult client for you. Ideally this will be done in
person – so a family member, roommate, etc. will work fine. However, if this is not possible, recruit a
classmate or other individual who will participate via distance technology (phone, video conference,
etc.).

Download the Comprehensive MDS that is posted in the Week 10 Module on Canvas. You will need to a)
Complete Section F, and b) Sign your name, title, section completed, and date of completion in Section Z
(Z0400). This Part of the Assignment is worth 15 points total.

There are two different ways you can complete this.


1. Fill the information in electronically and upload the full Comprehensive MDS to Canvas with
your sections completed. NOTE: You will need to use the “Fill and Sign” option in Adobe to edit
the document so that it saves your responses and lets you sign. Please verify your responses
have saved on the document before uploading to Canvas.
2. You can print out the required sections (no need to print out entire document) and fill them in
my hand. Upload to Canvas a scanned version or photo of your completed sections.
