Title : Amendment/Addendum: Certificate of Clearance

Item No. 4 of SOP No. HR-PR03.C (Amendment to SOP on Certificate of

Clearance) issued November 22, 1993 and effective November 30, 1993, which
requires an Undertaking for Payment of Account to be executed by a guarantor to
ensure the payment of account/s of a resigning/retiring employee which cannot be
established at the time of processing of clearance is hereby revoked.

Henceforth, disallowances or collectibles found after the issuance of the Certificate

of Clearance shall be collected from the concerned employee. The usual
procedure on billing and collection shall be followed. Should these actions prove
inadequate, legal means shall be undertaken.

All other provisions of SOP # HR-PR03 effective November 1, 1985 and its
amendments HR-PR03.B and HR-PR03.C which took effect on May 1, 1989 and
May 7, 1991 and November 30, 1993, respectively, not inconsistent with the
above shall still be in effect.
