ASSESSMENT
1. Determine current vital signs, level of
consciousness.

2. Check medication order and compare


against doctor’s order.

3. Make sure that the drug, dose, route and


time are the same in all aspects as in
medication order or card.

4. Check actions, nursing considerations,


adverse effects of the medication.
PLANNING
5. Wash hands.

6. Gather all equipment:


● Medication Tray
● Medication card
● Medication cup
● Medicine

7. Locate the medicine in the shelf or


medication cart and compare it with the
medication order or card.

8. Check the expiration date.


IMPLEMENTATION
9. Prepare medicines:
● If medicine is in liquid form
- Read the label of the bottle.
Hold the bottle with the label
against the palm.
- While holding the medicine
cup with the other hand,
place the thumbnail at the
mark that indicates the
amount of drug to be poured.
- Pour the drug at eye level.
Read the amount of
medication at the bottom of
the meniscus or the mark of
the thumbnail.
- Wipe the rim of the bottle
with paper and replace the
cap at once.
● If medicine is in unit dose
packaged, place the medicine in a
disposable cup.

● When tablet or capsule is in the


bottle container:
- Read the label of the bottle.
- Remove the cover of the

bottle and shake gently off
the prescribed number of
tablets or capsules onto the
cover.
- Place in the medicine cup.
Do not touch the tablet or
pills.

10. Check again the drug name on the label


and compare it against the medication
order.

11. Place the medication card and medicine


cup on the tray.

12. Take each of the remaining cards and


prepare other medication of the client
following steps 7-10.

13. Place water, drinking cups, straw on the


tray and proceed to the client's room.

14. Identify the client before giving the


medication by looking at the wrist band,
calling the client’s name and comparing
the name in the medication card against
the bed tag.

15. Assist the client to assume an upright


position.

16. Administer medication:


● Hand the medicine cup and water
to the client.
● Ask the client if he/she prefers to
take the medicine by hand or by
cup and one at a time or all at once.
● Do not leave the client until all
medications are swallowed. Never
leave medication at the client's
bedside.
EVALUATION
17. Never leave the client at once after giving
the medicines.

18. Assess the client on the side effects or


adverse effects of the medicine.

19. Discard disposable equipment and return


in place after washing remaining
equipment.

20. Perform handwashing.
