1.  Check the physician’s order.

2. Gather all equipment
 Medication
 Sterile 1 cc syringe
 Needle, gauge 25-27 (Kozier &
Erbs 2007)
 Cotton balls with alcohol or anti-
microbial solution
 Medication card
 Waste container for soiled cotton
balls 
 Hypodermic tray
IMPLEMENTATION
3. Perform hand washing
4. Check the label of medication carefully
against the medication card. Read the label
upon drawing from the cabinet before
withdrawing medication and
after withdrawing medication.
5. Withdraw medication from the vial or
ampule. Place the syringe on a medication
tray with the medication card.
6. Identify the client. Check the name of the
client at the door or bedside with the
medication card or address the client by
name.
7. Explain the purpose and nature of the
procedure to the client
8. Place the client in a comfortable position
and locate a injection site that is free of
lumps or nodules.
 Place the client on lying or side –lying
position with knees flexed if the site is in
the ventrogluteal muscles.
 The client may assume a lying or sitting
position If the site is the vastus lateralis.
 The client may lie on his back or sit in a
comfortable position with the arms relaxed
if the site is the deltoid muscle.
 Assist the client in assuming a prone
position
With his toes pointing inward, or a side
lying 
Position with the thighs flexed in front of
the legs if the site is the dorso-gluteal
muscles from the syringe.
9. Clean the area of the skin to be injected
using a firm, circular motion, moving
out from the central area with each
stroke.
10. Take off the needle guard; express any air
bubbles from the syringe.
11. Insert the needle at a 90-degree angle while
forming a cushion of the muscle by gently
pinching the skin together.
12. Pull back the plunger to check if the needle
is in blood vessels.
13. Inject the medication slowly.
14. Withdraw the needle quickly and massage
with an alcohol swab.
15. Discard the syringe in an appropriate
receptacle
16. Make the client comfortable.
17. Perform hand hygiene.
EVALUATION
18. Assess the site of injection
19. Monitor for signs and symptoms of side
effects and adverse effects of the
medicines to the client.
DOCUMENTATION
20. Document the medication given, the time
procedure was performed and the site of
the injection.
