This Method Statement Covers The Following:

1) Spading of pipe lines.


2) Open of bold & nuts and manhole cover.
3) Gas free of tank atmosphere.
4) Installation of ventilation and lighting.
5) Working in confined space inclusive Hydro water-wash
tank internal (confined space).
6) Removal of waste water from tank internal, disposal with
certified vendor.
7) Hand over to Client.
8) De- mobilize materials and equipment.

a) Blind, on the tank side, all lines to the tank except the water draw off line (carry
Out by, Client).
b) Disconnect all electrical power to all portions of the tank and follow the lock–out
Tag-out procedures. (Carried out by Client).

2. Opening of bold & nuts And manhole Cover

a) Trained personnel shall service nut and bolts one at a time.


b) Dip tray and spillage contaminant shall be placed under the manhole
c) Nuts and bolts servicing shall be stopped, where there is a leak in any flange and they will be
tightened back. Client shall be informed immediately
d) All equipment and ventilation be turned off at least 15min before any gas testing is carried out

e) the manhole shall put on full faced mask with 3M 6006 cartridge and 4 in 1 gas monitor the
manhole covers shall be removed by 2 men each with 01 additional man employed to help lower
the cover. A drip tray shall be placed beneath the manhole to address any residual product.
f) Wrench holders and non-spark tools shall be utilised during the opening of nuts and bolts.
g) One bolt shall remain in place after the manhole cover open and to be bonded with earthing
cable and supported by I additional man shall the manhole cover be lowered and removed to a
safe area.

i) The manhole cover shall be opened one at a time in the same manner

3. Gas free of Tank atmosphere

a) Is large enough and so configured that an employee can bodily enter and perform assigned work;
b) Gas free test and Permit Required Confined Space.
c) Contains or has a potential to contain a hazardous atmosphere.
d) Atmospheric oxygen concentration below 19.5 percent or above 23.5 percent.
e) Any other atmospheric condition that is immediately dangerous to life or health.

4. Installation of ventilation and lighting

a) The positioning of the ventilation fans shall comply with the ventilation plan.
b) Personnel to don all required PPE according to the Assessment in Risk Assessment.
c) Set up sufficient lighting.
e) Use class 1 DZ (IS proof) lighting for confined space.
f) NO POWER cords must be placed above ground by using of S hook.

5. Working in confined space inclusive Hydro-wash tank internal (confined space)

NOTE: The manhole watch shall note the temperature within the tank and specify the maximum
allowable time for work within the tank as stipulated in the heat stress Management plan. AN
EMERGENCY DRILL SHALL BE CARRIED OUT AT LEAST ONCE WITH ALL

PARTICIPANTS AND THE RECORD DISPLAYED BY THE SIDE OF THE MANHOLE
ENTRANCE.CYC supervisor shall brief the emerging procedure to all new workers when have not
attended the drill exercise. CYC supervisor shall brief the emergency procedure to all new workers
when have and attended the drill exercise.

a) The maximum number of entrants shall be as per the ventilation plan.


b) Manhole entry point must be clear from obstruction before allowing entry.
c) One person to monitor and record down the checklist every 30 mins.
d) He will ensure that the manhole entry point is cleared from any obstruction before allowing
entry.
e) The manhole watch shall maintain an entry register and SHALL NOT BE ASSIGNED
OTHER TASKS WHILE PERFORMING HIS DUTY OR LEAVE HIS POST UNLESS
DULY REPLACED BY A TRAINED AND COMPETENT REPACEMENT MANHOLE
WATCH.
f) The entrants shall be use supplied breathing air or half faced respirator with 6006 cartridge
subject to the Gas test results and approval before downgrading to full faced respirator with
3m 6006 cartridge.
g) A team of 2 men each shall be utilised with the hydrant hose for initial washing and a final
wash with a Hydro water - wash for tank internal.
h) Hydro-jet machine for the hydro-jetting of the tank internal. Any entry. Priority shall be
given to cleaning the area nearest to the entry point to clear any residual.
i) Obstacles and sumps shall be identified, marked and barricaded before allowing cleaning to
begin from inside the tank. Suitable and sufficient lighting shall be placed within the tank
before commencing cleaning.
j) The entrants shall update and take cue from the site supervisor before commencing
placement of the crib stack.
k) Each workgroup consisting of maximum 4 workers is required to carry 4 in 1 gas monitor.
l) Prior to entry into the tank, all entrant has to ground themselves to avoid static.

a) Wilden pump will be placed at flat ground

b) 1 person to monitor wilden pump.

c) Waste water will be dispose at the client designated area.

e) Provide drip tray below wilden pump.

f) R pins and whip arrestor to be secured on all hoses connection where required.

7) Handover

a) After tank internal cleaning is completed supervisor will invite CLIENT and operational
Personnel for tank inspection

b) After tank internal inspection is completed Supervisor will hand over the tank to the
CLIENT and complete and sign off on the tank acceptance checklist.

a) The site supervisor shall ensure that all personnel and EQUIPMENT are accounted for before
leaving the site and that housekeeping has been carried out. CONTAMINATED ITEMS ARE TO
BE DISPOSED OF IN SUITABLE BINS.
