MEMORANDUM OF AGREEMENT

This Agreement made and entered into this _______ of __________, 2022
and between:

IMPERIUM TRAINING CENTER INC., a private institution and represented


herein by its Head Administrator, Leonardo A. Estrella Jr with principal office
at F. Sario St. Brgy. Ermita, Paete, Laguna hereinafter referred to as “FIRST
PARTY”

AND
Judy-Ann N. Magsino of legal age, resident of Sitio Looban, Brgy Nanguma
Mabitac, Laguna as Admin Staff hereinafter referred to as “SECONDARY
PARTY”.

WITHNESSETH

WHERES, the FIRST PARTY is a private institution engaged in technical and


vocational courses and short term courses and information technology;
WHEREAS, the FIRST PARTY is in need of qualified Part-time Office Staff in
the training center or in any designated place in CALABARZON to perform the
same for the FIRST PARTY and the SECONDARY PARTY has accepted the
said offer, subject to the terms and conditions hereinafter set forth;

NOW THEREFORE, for and inconsideration of the foregoing premises, BOTH


PARTIES hereby mutually agree as follows:

ARTICLE I

SCOPE OF WORK

1. The SECOND PARTY shall be served as Office Staff to the FIRST PARTY
2. The SECOND PARTY shall render his/her services during the duration the
office hour assigned for the benefit of the FIRST PARTY, its and/or
employees.
3. The SECOND PARTY shall perform all office staff task and
duties/responsibilities and all activity to the training center of the FIRST
PARTY

ARTICLE II

AMOUNT OF CONTRACT

1. For consideration of the faithful and satisfactory performance by the


SECOND PARTY, the FIRST PARTY shall pay the SECOND PARTY the sum
of Eight Thousand Pesos (8,000) Monthly.
ARTICLE III
DURATION OF THE CONTRACT
1.This agreement shall be or the Start on June 16, 2022 up to July 16, 2024
set by the Officers of training Center.
1. BOTH PARTIES may seek to extend or reduce the period stated above
upon written notice of at least thirty (30) days prior to the extension/reduction
sought by either PARTY.
2. Should the SECOND PARTY fail to complete the services required within
the agreed period, the SECOND PARTY shall be liable for liquidated
damages at the rate of one-tenth (1-10 of the percent (1%) of the contract
price for each day of delay. In this case, the FIRST PARTY reserves the right
to hire another trainer and any expense incurred shall be charged against the
SECOND PARTY.
IN WITNESS WHEREOF, the PARTIES have hereunto signed this
agreement on the date and place first above written.

MEMORANDUM OF AGREEMENT

This Agreement made and entered into this ___ day of November , 2019 and
between:

IMPERIUM TRAINING CENTER INC., a private institution and represented


herein by it’s President, Alfonso S. Cabansagan with principal office at Purok
7, Barangay Caballero, Palayan City, Nueva Ecija hereinafter referred to as
“FIRST PARTY”

AND
BALAJADIA S. VILLAMOR of legal age, resident of 79 PUROK 6,SAN
NICOLAS ARAYAT PAMPANGA as Training center Instructor of HILOT
WELLNESS MASSAGE NC II hereinafter referred to as “SECONDARY
PARYT”.

WITHNESSETH

WHERES, the FIRST PARTY is a private institution engaged in technical and


vocational courses and short term courses and information technology;

WHEREAS, the FIRST PARTY is in need of qualified Part Time Trainer and
Instructor to conduct training to the trainees in the training center or in any
designated place in CABALLERO to perform the same for the FIRST PARTY
and the SECONDARY PARTY has accepted the said offer, subject to the
terms and conditions hereinafter set forth;

NOW THEREFORE, for and inconsideration of the foregoing premises,BOTH


PARTIES hereby mutually agree as follows:

ARTICLE I

SCOPE OF WORK

4. The SECOND PARTY shall conduct training for enrollees of FIRST PARTY

5. The SECOND PARTY shall render his/her services during the duration of
the course assigned for the benefit of the FIRST PARTY, its bonafide
students and/or employees.

6. The SECOND PARTY shall supervise/manage training to the trainees in


the training center of the FIRST PARTY

ARTICLE II
AMOUNT OF CONTRACT

2. For consideration of the faithful and satisfactory performance by the


SECOND PARTY, the FIRST PARTY shall pay the SECOND PARTY the sum
of One Hundred Twenty Five Pesos an hour.

ARTICLE III

DURATION OF THE CONTRACT

3. This agreement shall be or the period of OCTOBER 10 ,2019 up to


OCTOBER 10 ,2020/ whole training program set by the Technical
Education and Skills Development Authority (TESDA)

4. BOTH PARTIES may seek to extend or reduce the period stated above

CONTACT OF LEASE

KNOW ALL MEN BY THESE PRESENT

This CONTRACT OF LEASE is made and executed at the Palayan City ,


Nueva Ecija this day of 5th June 2018, by and between:

Mr. Alfonso S. Cabansagan jr. of legal age with principal address of 15


Dama de Noche Las Pinas ,Manila Purok 7, Owner of private land at
Barangay Caballero, Palayan City, Nueva Ecija with a land title no. 049-
2018000210 LOT NO. 111 PLAN NO. NG -130 (PLS-378) hereinafter
referred to as “LESSOR”

AND

IMPERIUM TRAINING CENTER INC., a private institution and represented


herein by it’s Administrator ,Leonardo A. Estrella jr with principal office at
Purok 7, Barangay Caballero, Palayan City, Nueva Ecija hereinafter referred
to as “LESSEE”

WITHNESSETH
That the LESSOR is the owner of land title no. 049-2018000210 LOT NO.
111 PLAN NO. NG -130 (PLS-378) located at Purok 7, Barangay Caballero,
Palayan City, Nueva Ecija consisting approximately 2,700 square meter and
hereby lets and leases unto the LESSEE the said area and the lessee hereby
accepts in lease from the LESSOR said leased premises subject the
following terms and condition .to wit.

1. That the term of this contract shall be for a period of (10) Ten years to
commence on October 2,2018 and to expired on November 2,2028 renewable
for another year, at the option of the LESSEE:

2. That the LESSEE shall give the LESSOR a notice of his intention to renew
the lease in writing not less that fifteen (15) days from the expiration of the
lease or renewed lease;

AMOUNT OF CONTRACT

For consideration of the faithful and satisfactory by the lessee, the lessee
shall pay the lessor the sum of ten thousand Pesos a month for ten (10)
years.
