AFFIDAVIT OF LOSS

I, ELIZABETH MADARANG IGNACIO, of legal age and

residing/with address at Mapaya 1, Sitio Ilocandia, Miembro
Residence, San Jose Occidental Mindoro, under oath, state:

1. THAT, on December 2019 at EMI Enterprises due to typhoon

URSULA that floods the following documents;
A. Original Books of Accounts:
- General and Special Journals
- Subsidiary and General Ledgers
B. Expense Vouchers and Purchase Invoices
C. List of Inventory

2. THAT, I exerted diligence efforts to locate the lost

documents said above but to no avail and I therefore
considered the same as lost and cannot be found;

3. THAT, I am executing this Affidavit to attest to the truth of the

foregoing facts and to use the same for whatever legal purpose
it may serve.
