Sub: Documents required for updating KYC details who holding shares in Physical mode

R e f: C i r c u l a r N o . SEBI/HO/MIRSD/MIRSD_RTAMB/P/CIR/2021/655 dated 0 3 November 2021 on

Common and S i m p l i fi e d Norms for processing Investor's service requests.

Refer to the aforesaid circular issued by SEBI on "Common and Simplified Norms for processing investor's service

requests by Registrar and Transfer Agent (RTA) and norms for furnishing PAN, KYC details and Nomination", we

are requesting the shareholders who are holding shares in physical mode to furnish the following mandatory

documents:

1) A copy of Valid PAN i n c l u d i n g PAN of Joint' S h a r e h o l d e r s duly linked with A a d h a a r and KYC d e t a i l s .

2) Nomination Form with Specimen Signature and self-attested PanlAadhaar card copy of the Nominee.

3) Postal Address with Pin code, E m a i l id and Mobile Number.

4) Bank Account d e t a i l s ( B a n k Name, B r a n c h , B a n k Account N o , IFSC Code and M I C R code) with

Cancelled Cheque leaf.

Shareholders who are holding shares in physical mode are required to furnish all above details immediately, failing which

all such p h y s i c a l folios shall stand frozen with effect from A p r i l 1 , 2 0 2 3 . While furnishing the PAN please ensure that

the same had already been linked t o A a d h a a r and if not yet d o n e , please update same i m m e d i a t e l y, or any other date as

may be specified by the Central Board of Direct Taxes ( C B D T ) otherwise such Folios s h a l l stand frozen with effect from

April 1 , 2023.

You are requested to send us duly completed required Mandatory forms as stated below along with self- attested copy

of proofs (PAN & Aadhaar cards and Original Cancelled cheque leaf with date.

F u rth e r please note that the Shareholders held Securities under frozen folios shall avail service request from the

RTA only after f u rn i s h i n g the complete KYC details. And it shall be referred by RTA and the Company to the

Administering Authority under the "Benami Transactions ( P ro h i b i t i o n s ) Act, 1988 and or Prevention of Money

Laundering Act, 2002", if they continue to remain frozen as on 3 1 . 1 2 . 2 0 2 5 .
