CONTRACT OF LEASE

KNOWN ALL BY THESE PRESENTS

This CONTRACT OF LEASE made and entered into by and between

Mrs. Loreta D. Angel, representing Angel’s residence, legal age, married, Filipino
citizen and wit postal and residence address at Sumagui, Sta. Maria, Ilocos Sur, hereinafter to
a LESSOR.

PCPT JOEL P DASARGO, Officer-In-Charge, of legal age, married, Filipino


citizen and with office address at Maynganay Sur, Sta. Maria, Ilocos Sur, hereinafter referred
to as a LESSEE.

Whereas, the LESSOR is the owner and sole, exclusive and absolute possessor of
certain house located at Sumagui, Sta. Maria, Ilocos Sur.

Whereas, the LESSEE has offered to lease a portion of said property consisting of 30
square meters from the LESSOR and the latter has accepted such offer under the following
terms and conditions, to wit:

1. That the term of the lease shall be for six (6) months commencing from July 1,
2021 and to terminate on December 31, 2021.
2. That the monthly rental fee of the leased premises shall be P 500.00 and P
146.00 for maintenance;
3. That the LESSEE upon the execution of this contract shall make one (1)
month deposit to the LESSOR in the sum of Seven Hundred Fourteen Pesos;
4. That the LESSEE shall pay the LESSOR at the latter’s address above-
mentioned, the monthly rental on or before 30th day of each month, without
need of any demand.
5. That the LESSEE December introduced or build light materials with the
leased premises with the prior consent of the LESSOR, provided, however,
that all alterations or improvements of whatever nature introduced by the
lessee shall form integral part of the leased premises and shall become
exclusive property of the lessor upon the termination of the lease without need
of reimbursement for the cost; thereof;
6. Charge of bills of water, electricity, telephone and other utility services in the
premises as well for the repairs in the utility system shall be shouldered by the
lessee;
7. That the LESSEE shall be responsible for the observance of sanitary and
electrical regulations required and imposed by the Municipal Government
authorities regarding the use of habitation of the leased premises;
8. That in case the LESSEE shall leave or abandon the leased premises
continuously without the consent of the LESSOR for a period of at least thirty
(30) days, the LESSOR shall have the right to enter the premises and repossess
the same or least it to another, with authority to take all the properties and
belongings of the lessee at the expense of the LESSEE,
9. That the LESSEE agrees to vacate the premises peacefully, vacant,
unencumbered and in such condition as herein above covenanted, after the
termination an cancellation of this contract for whatever cause;
10. That this contract December be renewed or extended by mutual consent of the
LESSOR and the LESSEE, provided, however, that eight party shall have the
option to terminate this contract anytime even before expiration by giving at
least thirty (30) days’ notice of such option. In case the LESSEE exercises this
option, then the lessee shall peacefully vacate the property without the
necessity of any judicial order after the expiration of the said thirty (30) days’
notice;
11. That the LESSEE is expressly prohibited to sublet the lease premises to any
one, without the express consent of the LESSOR in writing;
12. That violation of any of the above terms and conditions will produce ipso
facto the rescission of this contract lease.

ACKNOWLEDGEMENT RECEIPT
THIS IS TO ACKNOWLEDGE RECEIPT from PCPT JOEL P DASARGO,
Officer-In-Charge of STA. MARIA POLICE STATION the amount of 500.00
