Republic of the Philippines if Department of Education REGION I SCHOOLS DIVISION OFFICE DAGUPAN CITY CONTRACT OF AGREEMENT THIS AGREEMENT made this 13 day of May 2022 between the Department of Education, Division of City Schools, Dagupan City (hereinafter called the “Entity’) NUTRIDENSE FOOD MANUFACTURING CORPORATION (hereinafter called the “Contractor') WHEREAS, the Entity is ardent that the Contractor execute Three Million Four Hundred Sixty Thousand Eight Hundred Sixty Pesos (Php 3,460,860.00) (hereinafter called “the Works’) and the Entity has accepted the bid for Three Million Four Hundred Sixty Thousand Eight Hundred Sixty Pesos (Php 3,460,860.00)by the Contractor for the execution and completion of the following Works and the remedying of any defects therein: NUTRITUOUS FOOD PRODUCTS - SCHOOL-BASED FEEDING PROGRAM FY 2022 NOW THIS AGREEMENT WITNESSETH AS FOLLOWS: 1) In this Agreement, words and expressions shall have the same meanings as are respectively assigned to them in the Conditions of Contract hereinafter referred to. 2) The following documents shall be attached, deemed to form, and be read and construed as part of this ‘Agreement, to wit: (@) PhIIGEPS Invitation to Apply for Eligibility and to Bid; (©) Bid Data Sheet (©) General and Special Conditions ofthe Contract (@) Specifications; (©) Bid form, including all the documents/statements contained in the Bidder's bidding envelopes, as annexes; (9. Eligibility requirements, documents and/or statements; (a) Notice of Award of Contract and the Bidder’s conforme thereto; (@) Other contract documents that may be required by existing laws and/or the Entity.
