After Activity Report On Investigation of Fire On Train Incident
1. Please be informed that at exactly 8:16am, October 10, 2021, 14 personnel from
DOTr-MRT3, TESP, and Oriental Consultant Global (OCG) attended the joint
inspection of Index 5 with train composition 57-66-32, the involved train on Fire
Incident transpired in between Buendia and Guadalupe Station.
2. The said activity was started by a meeting presided by Dir. Michael J. Capati. The
purpose of the inspection and investigation is to find out the root cause of the fire
incident of Car no. 32. Dir. Capati instructed the SUMITOMO President Gaku Kondo
and Mr. Ardie Datuin to create investigation team. The said team will submit a daily
progress report to the OOD on the status of investigation. Dir. Capati suggested that
8 passengers involved in the incident shall be given a perk of free ride until
December 31, 2021.
3. The Joint Investigation Team was composed of the following representative from
DOTr-MRT3, SUMITOMO MHI TESP, and OCG.
4. At around 9:22pm, Engr. Oscar Bongon together with the SSU personnel, Transport
Division, OCG Consultant and TESP (Maintenance Provider) proceeded to Track 24
for visual inspection.
6. At around 2:20pm, the investigation team conducted inspection activity at Car no. 66
and Car no. 57. They inspected electrical box, battery box, chopper box, reactor box,
battery connecting switch, brake valve, and traction motors. Furthermore, Mr. Chris
Saloma, TESP TRS Senior Engineer, pulled the MESSMA cards of Car 66 and Car
57 for reading.
7. At around 3:40pm, the Car no. 66 and Car no. 57 underwent forward and backward
drive testing. At around 3:59pm, the investigation team inspection was terminated.
According to Mr. Rodel Solomon, TESP Executive Senior Engineer, the Car no. 66
and Car no. 57 will undergo low speed tomorrow, October 11, 2021.
